Justifies patient’s plan of care using appropriate resources s S Student Reflection (Thoroughly answer all questions) How has this clinical rotation helped to define your role as a nurse? Please explain. This clinical experience was an eye opener for the realty of our need for higher standards regarding this level of care. The nurses on this unit have a tremendous load distributed between far too few to be able to give quality care to these high need patients. Since geriatric care is the specialty that I would like to pursue, it is quite alarming and disheartening. Yet, it just goes to show how I can make a difference as a nurse. Yes, it is amazing given the tons of laws and guidelines that have been developed around LTC that it could still be so poorly executed. I am glad that the group was able to see the “tremendous load” and staffing shortage and still appreciate the fact that staff really does try to do a good job. Which objective did you accomplish during this rotation? How did you do it? The patient that I was assigned was an 85-year-old female that had been recently assigned to hospice care, therefore, she was only in need of palliative care. While palliative is somewhat limited, I believe the objective that I accomplished during this rotation was providing patient-centered care with assistance. Having limited medical experience, I am learning more and more of how to recognize the needs of my patients. For example, with this patient, her ability to communicate her needs was limited due to her speech difficulties. Therefore, Professor Taylor was able to make me aware of certain concerns she has noticed over the past month and I was able to attend to these needs with her assistance. For example, replacing the water on her oxygen humidifier and finding a swab for her dry mouth. With the assistance of my fellow nursing students, we were able to clean up the patient and make her more comfortable. This may have taken quite some time, otherwise, given the understaffing. I was also able to assist other patients with ADLs and comfort measures as they arose. Yes, I think we were definitely able to make a difference in her day. What are your goals for your next clinical rotation? My next clinical rotation is acute care. For that rotation, I want to take what I have learned from each of the clinical experiences that I have had thus far and apply it. Specifically, making use of the assessment and care skills I have acquired. I hope that as I become more knowledgeable and get more hands-on practice, my confidence and abilities to provide quality care will increase. Good goals. Assessment, and the ability and dedication to act on our findings really define us as nurses. Faculty Comments/ Student Learning Goals (Goals must be established even if performance is satisfactory. Even though this group didn’t really get very far into the Charts and PPC until our 2 nd day, you all did a very good job of combing through that information and identifying things we could be doing better for these residents. As your on-going goal, keeping what you learned about how info was being communicated – or not communicated – I suggest working on assessment and reporting. We saw a lack of both of these skills in our LTC setting, but I think that once you get into the Acute care setting you will have more opportunity. Review and practice. What do you need to learn about your patient on this shift today, what do you need to do about it and what is most essential to pass on in report.
