Dear STEM Students,

We are Grade 12 GAS students of Manaoag National High School, who are currently
doing a research on Implications of Body Shaming: Preliminary evidence for a further
subjective experience of self-objectification in Grades 11 and 12 STEM students of
Manaoag National High School.

The study is a major requirement for Practical Research 2 subject. In this regard, we
would like to solicit your permission to survey you to learn more about body shaming.

If you choose to participate in this study, we will schedule a time to meet with you at
your most convenient time. During this time we would like to ask questions concerning if
the students are being conscious in their body image and what are the efforts they
make for their body to be healthy and fit.

This survey will take approximately one hour. With your permission, we would like to
survey in order to focus on the conversation. Only the group will have access to the
questionnaire. The questionnaires will be transcribed and we will remove any identifiers
during the transcription.
