In line with this COVID-19 pandemic the world is experiencing, the student firmly
believes that an alternative means of enrolment procedure should be done in order to
cater the academic needs of the student.
I, Exan John DF. Carpio, a current second year student taking up Bachelor of Science
in Physics, hereby authorize Prof. Karla B. Rivero to direct all the necessary process
of enrolment for the academic year 2020-2021 (1 st semester). The success of the
enrolment will then secure a slot for the student on all of the loads for the THIRD YEAR
LEVEL as indicated in the program’s prospectus, provided that the student complied
