Sir/Ma’am,

At around 6:15 pm April 25, 2020 when I was finished cooking the food (dinner) of
Bahay Pag-asa clients, I got permission to the Civil Security Unit (CSU) on duty Jerome Diego
and Home Care House Parent on duty Ma’am Christine Fae Valiente. I went home at Brgy. 45
Tangid Laoag City to take a bath and change my cloth due to the undersigned was on duty for
twenty four hours (24 hours April 25, 2020) at Pag-asa Building.

When I was drove back at around 6:45 pm along the highway Ma’am Christine Fae
Valiente called me up and telling the incident, that the client KAIRUZ JAMES CRUZ is
absconded. The undersigned immediately report to Barangay Tanod, Front liner on duty at Brgy.
47 Bengcag Laoag City and Brgy. 48A Cabungaan Front liner on duty beside of Jessica Store to
inform and report the incident that one of our client is absconded and immediately inform to my
Social Welfare office Ma’am Evelyn Calubaquib at around 7:12 pm thru cellphone that our
client stated above is absconded.

The incident were immediately conducted search operation with CSU personnel’s, PNP
officers and Brgy. Officials and Tanod’s , but we failed.
