REMINDERS FOR SUBMITTING THE


PEER EVALUATION FORM
▪ The Peer Evaluation Form (PEF) is for each member to evaluate his (her)
performance and those of the other members of the group. The Peer Evaluation
Form will be accomplished and submitted to the Thesis Mentor twice (1) Start of the
Term up to the Midterm (2) Midterm up to the Finals.

▪ Each member of the thesis group will have to complete one Peer Evaluation Form.
The member has indicate the names of the three other members of the group on the
designated columns.

▪ Members of the thesis group should remember to:

▪ Indicate your complete name and the complete name of the other members of the
group, the name of the thesis mentor, and the title of your study. Sign the form,
place it inside a long white envelope and seal it.

▪ LABEL THE SEALED LONG WHITE ENVELOPE WITH “YOUR COMPLETE NAME
(LAST NAME, FIRST NAME, MIDDLE NAME), THSPSMA (PEER EVALUATION: START
OF THE TERM UP TO THE MIDTERM”

▪ Please submit your accomplished Peer Evaluation Form to your


THESIS MENTOR.

▪ Your Thesis Mentor will use this as a guide in determining the contribution of each
member of the group in the completion of the THS1PSM output. The points presented
in this form could influence the grade that the Thesis Mentor will give each member
of the group.
