AFFIDAVIT OF LOSS

I, FERDINAND R. BONIAO, Filipino, of legal age, single and a resident of

Telaje, Tandag City, Surigao del Sur, Philippines, after having been duly sworn
to in accordance with law, depose and state:

1. That I am a registered owner of a parcel of land located in

Magugpo, Tagum City, Davao del Norte;

2. That as such, I was issued by the Registry of Deeds, Tagum City,

with a Transfer Certificate of Title with Number: T-244107;

3. That because I maintain several places where I have put up

businesses, I can no longer remember where exactly I have kept the said Title. I,
however, checked all my cabinets and vaults in all my branches; yet, it is
nowhere to be found;

4. That I have already exhausted all earnest efforts to locate my Title;

unfortunately, my efforts were in vain;

5. That I hereby attest that my Title was not in the hand of any person,
who may be a buyer, donee or a mortgagor; and

6. I am executing this Affidavit of Loss to attest to the veracity of the

foregoing and to support my application for new owner’s duplicate of my Transfer
Certificate of Title.
