The undersigned, JAMMYGIRL MAKABALIGLIOG hereby applies to have the land hereinafter
described brought under the operation of the Property Registration Decree, and to have the title thereto
registered and confirmed:

AND DECLARE . . . . .

1. That the applicant is the owners of the land (by virtue of inheritance), together with the building and
improvements thereon. The said land, consisting of 25 hectares parcels are situated, bounded and
described as shown on the plan and technical descriptions attached hereto and made a part hereof;

2. That said land at the last assessment for taxation was assessed at P 20,202,020.20 Philippine currency,
and the buildings and other improvements at P 8,567,890.00 Philippine currency.

3. That to the best of my knowledge and belief, there is no mortgage or encumbrance of any kind
whatsoever affecting said land, nor any other person having any interest therein, legal or equitable, or in
possession;

4. That the applicant has acquired said land in the following manner: inheritance from a common
ancestor on which has been occupying the subject land for 60 years from 1961 up to present;

5. That said land is occupied solely by the applicant;

6. That the name in full and addresses, as far as known to the undersigned, are as follows:
JAMMYGIRL ESPINOSA MAKABALIGLIOG, Brgy. Untaga, Alicia, Bohol 6314;

7. That the applicant is married to PROCOPIO MAKABALIGLIOG;

8. That the applicant's full name, age, citizenship, residence, and postal address are as follows:
JAMMYGIRL ESPINOSA MAKABALIGLIOG, 22 y/o, Filipino, Brgy. Untaga, Alicia, Bohol 6314;

9. That the following documents are attached hereto and made a part hereof:

a) Plan in tracing cloth of land attested and duly certified by the Regional Technical Director;
b) Technical descriptions certified by the Regional Technical Director;
c) Geodetic Engineer’s certificate; and
d) Certificate of the Municipal Assessor of the assessed value of the land and its last assessment for
taxation.

WHEREFORE, it is respectfully prayed that the Register of Deeds Tagbilaran City, Province of
Bohol issue an original certificate of title of the land located at S., along line 1-2, by Lot No. 2277 Sitio
Bays-iring, Brgy. Untaga, Alicia, Bohol 6314 Philippines registered in my name, Jammygirl M.
Makabaligliog as petitioner and applicant. Signed at Tagbilaran City Province of Bohol this 20th day of
October, in the year twenty hundred and twenty.
