AFFIDAVIT OF GUARDIANSHIP

I, JULIE DUMAGUING TAWANG, Filipino, of legal age, widow, and a resident of

IC 155 A Betag, La Trinidad, Benguet, Philippines, after having been duly sworn in
accordance with law, hereby depose and state:

1. That I am the Legal wife of the late JON JUN MIGUEL TAWANG who is the
sole heir of his biological mother LISSING MIGUEL POOL, who is my
Mother-in-Law;

2. That during my marriage with JON JUN MIGUEL TAWANG, we begot four
(4) Children;

3. That for being their biological mother, I am the legal guardian (hereinafter
“legal Guardian”) of our children, all minors, namely:

a) NICOLE ANGELO TAWANG;

b) JUSTIN KANE TAWANG;
c) TRYXYN JAROLD TAWANG; and
d) MATTHIEW JUIEL TAWANG.

4. That there is no other judicially appointed guardian over the person or

property of the our said minor children;

5. That I was never been divested of parental authority over the persons and or
properties of the said Minors and that I was never been convicted of any
crime which carries with it the penalty of civil interdiction;

6. That I have not excluded any other legal Guardian of the persons or
properties of the Minors;

7. As such, I execute this Affidavit to certify or attest to the truth of the foregoing
facts and for whatever legal purpose that this Affidavit may serve.

IN WITNESS WHEREOF, I have set our hands this 16 th day of November 2020,
in the Municipality of La Trinidad, Benguet, Philippines.
