Data Gathering Instrument

The instrument used in this study was a researcher-made survey

questionnaire as the primary tool for the data gathering. The survey

questionnaire was originally formulated by the researcher as presented in

Appendix B. The said instrument was reviewed by Dr. Marciano Melchor to

assess clarity of directions and items, presentation and organization of items,

suitability of items, adequateness of items per category or indicator, attainment

of the purpose, objectivity, and scale and evaluation rating scale. Aside from

assessing the questionnaire, the evaluator also recommended some items to be

added, improved and replaced from the original questionnaire. The

questionnaire was designed to assess the level of students’ awareness on the

occurrence of bullying in Agro-Industrial Foundation College of the Philippines.

Subsequently, the improved survey questionnaire was pre-tested to 15

respondents, who were not included among the actual respondents, to ensure

consistency of responses collected by the instrument. It would also collect

relevant information and would bring to light its strengths and weaknesses.

The collected results of pre-tested questionnaire had undergone a reliability

test where it gives a Cronchbach’s Alpha equal to 0.977, which indicates high

level of internal consistency for the scale as shown in Appendix E. The

responses and feedbacks in the pilot survey were used to improve final

research questionnaire prior to its administration. The questionnaires were

administered through direct and face-to-face conversation with the

respondents.
The administered questionnaire used a Five-point Likert Attitudinal Scale

as response to the questions to assess the knowledge about bullying, factors

that causes bullying and effect of bullying among Senior High School students

as shown in Table 1:
