SUBJECT: ON-LINE SUBMISSION OF THE LEARNERS’ PROFILE FOR THE

PROPOSED
LIMITED FACE-TO-FACE

1. In preparation for the proposed limited face-to-face transition classes for the academically
challenged learners, this office will be conducting an ON-LINE SUBMISSION OF THE
LEARNERS’ PROFILE. This aims to determine the number of classes to be organized by
grade level and per learning area.

2. In view thereto, all school heads are enjoined to conduct profiling on the following such as but
not limited to:

2.1. No. of learners who are academically challenged or not;

2.2. No. of learners who are academically challenged by learning area
2.3. No. of learners with and without health conditions;
2.4. No. of learners with or without instructional support at home;
2.5. No. of learners with or without parents’ consent; and

3. To determine learners who are academically challenged or not will be based on the result of
output of the activities, summative assessment or formative assessment;

4. Attached herewith is the prescribed template for the school use. However, for the on-line
submission, please log-in to http://bit.do/limited-ff-profiling. The submission will be on or
before December 4, 2020 at 5PM.

5. Widest dissemination of this Memorandum and strict compliance is hereby directed.
