School-Based MID-YEAR PROGRAM IMPLEMENTATION REVIEW

June 30, 2023

I. Introduction
Program Implementation Review helps collect data and assess the programs and projects
implemented per school, furthermore, The activity aims to:
a. assess the implementation of the major Programs, Projects, and Activities (PPAs) of the
schools per district vis-à-vis the committed outputs and targets for FY 2022;
b. identify issues, bottlenecks and concerns that need to be addressed by the management or the
program implementers to ensure achievement of targets and deliverables for the remaining
quarter of FY 2022;
c. identify courses of action to be undertaken and provide necessary technical assistance to
address concerns and bottlenecks in the implementation of PPAs; and
d. share best practices of each District.

At the District Level:

a. Assess their Key Performance Outcomes per school for SY 2021-2022;
b. Determine quick wins and its contributing factors;
c. Determine the major challenges and its contributing factors;
d. In contributing factors, district shall classify the PPAs performance in terms of financial,
physical and timeliness; and
e. Identify issues and concerns and those needing decisions from the Top Management (DO
level, RO level, CO level).
Data shows the KPI for 3 consecutive years. We have 0 drop-out rate for this school year.
The data shows a good result since we have 100% Net Intake Rate and Retention Rate. Also, all
the learners were promoted to the next grade level.
The data shows the quarterly result of CMSS.
SEF Budget for 2023 is Php125,000.00 which was 100% utilized for the repair of ceiling for
Marcos Type Building, Division wall of Grade 2 & 3 and Office and Kindergarten wall.

11. Checking of DLPs/DLL every Monday
