Learning Activity 5

1. Relate your ideas on health care process to community health care process.
I believe that health care process is the same with the community health care process, only that in community health care process, you are dealing with a larger population.
Moreover, as in a larger scope, there are more aspects in the community. Thus, it will require more time and effort in assessing the overall health of the community.

2. What are the activities you or your groupmates have gone through from community assessment to conduct a health education?
Back in the community, as we conduct home visits and go through assessing the health of each member of the family, we make sure to educate them at the same time.
Whenever there are questions that we lack knowledge of, we address it directly with our community instructor and make sure to inform the family as soon as we go back to
their place.

3. This question is related from your previous learning activity. Did your organization encounter any conflicts? If so, how did you resolve these problems?
We encountered Task Interdependencies when works were divided by group and some members failed to comply leaving the group to fall behind the target time of
completing the presentation. We did the Collaboration where the uncompleted task was solved with our group’s effort. With equal cooperation and teamwork of the other
members, we were able to finish the work before the date of presentation and it turned out to be successful.

4. Scenario: You are a student nurse team leader to be rotated to Barangay Dimi Nahal. The previous group assigned to the said community was already able to
select the members of the core group. They have endorsed to your group that the next activity to be conducted is to make the core group to be more
knowledgeable and competent thru series of trainings and seminars. As a student nurse you know that prior to conducting trainings and seminars you should
first conduct Training Need’s Assessment to your participants. As the team leader and a member of the group, you are to construct a Training Need’s Assessment
Tool with at least 15 important topics or critical areas to be assessed.
