Proposed Price ---- Total cost of $2800.00 per year, payable @ $700.00 every three
months.

Note: All work to be done during normal business hours

Should performance of the above noted that any additional labor or materials be required to place the
equipment in the proper operational order, you will be notified and your approval obtained prior to our
proceeding with any additional work

The above referenced priced will be held firm for a period of (30) days from the date of this proposal. If

AC Units, Filter Changes, Exhaust Fan, ----- Done quarterly

- Change filters on floor heaters. From 1st - 14th floor

 - Change filters on carrier unit that supplies 1st floor
- Clean Samsung unit filter that supplies elevator room
- Clean coils check for , check refigerent charge, check elec.
- Change belts on exhaust fans 1 though 9

- Lubricate pump and bearings
- Check line strainers, pressure gauges, packing glands, mechanical seals.

Boilers on Roof
- Inspect boiler pressure vessel for possible
- Check hand valves and automatic feed equipment
- Check fuel piping for leaks
- Check auxiliary equipment operation
- Inspect burner, boiler and controls
- Check safety controls

If proposal is accepted Screens 4 Less will inform you of the schedule of when we will be coming
out , and what systems we will have maintenance done.

The following rates will apply to all mechanical repairs or services which are outside the scope
of this agreement.

Hourly rate for normal business hours M - F 8am - 5pm---------------------------- $65.00

Overtime------------------------------ $117.00

Sundays and Holidays---------------------- $169.00
