Subject: “HANDANG ISIP, HANDA BUKAS!” YOUTH FORUM

The Bureau of Learner Support Services – Youth Formation Division (BLSS-

YFD) together with the Public Affairs Service (PAS) will conduct the “Handang
Bukas, Handa Isip!” Youth Forum between August to September 2020.

The activity aims to:

1. Touch on the role of the youth sector in the delivery of verified information
on learning contiuity in the “new normal”;

2. Provide the youth sector with updated information on the Learning

Continuity Plan (LCP);

3. Deliver other relevant information in the education sector:

4. Gather feedback on the integration of Learning Continuity policies.

In this regard, all Officers of the Division Federation for both the Supreme
Pupil Government and Supreme Student Government are encouraged to attend the
online forum.

Details of the Forum will be relayed to Regional and Division Youth

Formation Coordinators and their respective school SPG/SSG Advisers, and will
assist in the registration and participation of participating Federation Officers.

CONSENT FORM

Handang Isip, Handa Bukas! Youth Forum and engage with them in a meaningful way
throughout the event. A set of guidelines and procedures on youth safeguarding have been
established and all participants are required to adhere to the following code of conduct.

In the context of the Handang Isip, Handa Bukas! Youth Forum , the organizers and
participants will:

• Treat young people with dignity and respect at all times;
• Be sensitive to young people’s age, gender, ethnicity, background, sexuality, religion, and
other important personal characteristics;
• Treat and engage with all young people as equal partners, despite of their personal
characteristics;
• Never participate in any behavior towards young people that may be illegal, unsafe and
abusive;
• Familiarize themselves with the youth safeguarding documents and procedures for the
event.

I hereby agree to adhere to the youth safeguarding code of conduct set by the organizers.

USAGE OF MEDIA MATERIALS AND OTHER DATA GATHERED

This form confirms that all participants agree to be recorded during Handang Isip,
Handa Bukas! Youth Forum event activities and permits the Organizers to use some or all of
their image/contribution/performance in any publication (including electronic publications
such as films or websites) created by or for the Organizers and to release this material to
DepEd Official platforms.

I hereby confirm that I understand and agree with the terms and conditions outlined
above regarding the usage of media materials in the Handang Isip, Handa Bukas! Youth
Forum.
