PURPOSE OF MEDICATION

Expected Pharmacological Action Therapeutic Use


Increased urine output Emergent need for rapid mobilization of fluid
K+ is excreted
Block absorption of sodium & chloride Pulmonary edema caused by heart failure
Prevents absorption of water Edema caused by liver, cardiac, or kidney disease
Causes extensive diuresis or hypertension is not responsive

Complications Medication Administration


Dehydration, hyponatremia, hypochloremia Oral, w. food to prevent GI effects.
Hyptotension
Ototoxicity if taking more than once daily, give
Hypokalemia second dose by early afternoon to
Decrease in HDL levels prevent nocturia & sleep loss
Increase in LDL levels
IV, administer slowly to prevent
ototoxicity
IM (intramuscular)
Contraindications/Precautions
Contraindications: clients w. no urine output
Precautions: clients w. DM, dehydration, electrolyte depletion, &

Monitor ECG if K+ levels drop
Digoxin toxicity can occur in the presence of hypokalemia below 3.5 mEg/L & notify provider
Concurrent use of antihypertensives can have addictive
hypotensive effect
Lithium carbonate blood levels can increase
NSAIDs decrease blood flow to kidneys

Self monitor BP & weight
(hypertension only)
Evaluation of Medication Effectiveness Report significant weigh loss,
Decrease in pulmonary or peripheral edema lightheaded-ness, GI distress
Weight loss Consume food in high K+
Decrease in BP Monitor for elevated blood glucose
Increase urine output levels (DM clients)
Decrease in calcium level Report manifestions of ototoxicity
