2. SCOPE OF WORK

2.1. General Description

 Excavation to meet the required invert elevation to construct or repair the existing structure
then to backfill after completion of the required scope of work with conformance with the
specification.

2.2. Plant and Equipment

Material

 List the type and approximate quantities to be used.


1. Backfilling materials (quantities will determine later)

 Explain how deliveries are handled on site and describe the logistic routines.

The deliveries will be by dump truck and deliveries will be according to the required
quantity in the site.

2.3. Site Planning & Preparation

Encroachment permit will be obtained first before starting any activity in the site. Once

the area is determined and staking is complete, the safety personnel will established the
perimeter barricade to warn others. If rerouting is necessary the safety personnel will
establish first the rerouting of access roads and install warning devices to warn passing
traffic in the area and they will also coordinate with other contractors working in the area
for proper coordination. The safety personnel will coordinate with the construction
supervisor for the entry and positioning of heavy equipment and establish a danger
zone so that safety personnel will install warning signs. Prior to excavation using heavy
equipment a manual excavation shall be done first if the area is a high risk area
(presence of underground facilities), once all underground facilities is secured then
equipment excavation can be implemented. All communication in the site will be by
phone and constant supervision will be implemented during the excavation in order to
avoid untoward accident.

2.4. Work Methodology

 Listed below is the activity in sequence for the excavation:


1. Staking of the area were to be excavated including the clearance for the slope according
to the depth of the excavation.
2. Establish perimeter warning sign and temporary detour road if necessary.
3. Commence manual excavation if underground cables or underground infrastructure are
present in the area, then install temporary support if necessary and warning sign to
protect, then commence mechanize excavation using an excavator .
4. Always establish a minimum 1 meter distance between your excavation limit and the
excavated materials and the side shall be slope to avoid caving in.
5. If the deep of the excavation exceeds 1.2 meters establish a benching with a minimum
width of 1 meter to accommodate the well point of the dewatering system then install and
commission the dewatering system.
6. Twenty four (24) hours after the dewatering system was commissioned and the water table
has been lowered to a depth of at least 300mm below the excavation level (RC Spec’s,
Sec. 02315 par. 3.03-A.20) commence the excavation to meet the required invert
elevation.
7. Commence sub-grade preparation by doing watering and compaction using a plate
compactor in order to meet the minimum required compaction density which is 95%.
8. Once the sub-grade passes the compaction test, proceed for the next activity.

 Listed below is the activity in sequence for the backfilling:


1. A Hold Point release shall be issued first before any backfilling activity can commence.
2. Backfill the area with maximum 300 mm compacted thickness then commence watering
and compaction to attain the minimum required compaction density which is 95%. All
testing shall be witness by the Royal Commission Engineer.
3. Once the compaction density was meet and passes, commence the next layer and repeat
the activity until backfilling is fully completed.

4. The excess materials shall be dispose to the designated area and then clean and remove
all safety warning sign.

2.5. Organisation, Responsibility & Supervision

 Construction Manager – will be the one to plan and coordinate all the activity in the site
and he will be responsible for coordinating with other contractor working in the area.
 Safety Personnel – will be the one to establish all necessary warning signs and barricades
establish the high risk areas in coordination with the Construction Manager and constantly
monitor the activity in terms of safety to all the workers. Furthermore he will give advice on
current and forecasted safety issues and concern.
 Site Engineer - will be the one to implement the instruction of the Construction Manager in
the site, he will also directly supervise constantly the manpower and equipment working in
the area and implement all safety regulation and precaution set by the safety personnel.
 Quality Control Inspector – Will be the one to inspect and monitor the repaired area before
recommending to RC Engineer for inspection.
 Supervisor – Will be the one to supervise and assign the works as per schedule.
 Technician – Will be the one to do compaction by the aid of compactor.

3. SAFETY AND HEALTH

3.1. Risk Assessment

 The following are the risk assessment for this activity:


1. Proximity of work in relation to public areas.
2. Existing site conditions and damage to public areas.
3. Traffic accident due to no barricade and warning signage.
4. Caving in of excavated materials
5. Equipment accident due to blind side and no spotter.
6. Equipment accident due to soft soil condition.
7. Damaged of underground cables or underground infrastructure.

 SECTION II – 200 SERIES (15: Personal Protective Equipment):


15.1 Personal Protective Equipment shall be considered only as a “last resort of
defence” against potential risks and hazards. Where so ever possible hazards
and risks are to be addressed and reduced by other means.
15.2 Based upon the findings of 3.1.3 Section 1 and 11 above the contractor shall
ensure that adequate stock of Personal Protective Equipment are readily either
available on site or issued to personnel. PPE shall be considered as consumable
items.
15.3 All PPE utilised shall be manufactured to a recognized international acceptable
standards.

3.5. Information to Personnel

 All the manual workers that will be working in the site will be informed and reminded by
conducting morning tool box meeting in order to continuously remind them of the existing
safety risk and it will be handled by the designated site supervisor and sometimes with the
presence of the safety personnel.

3.6. Emergency Procedures

 The following are the safety procedure that will be implemented in case of accident:
1. In case of traffic accident or person hit by the equipment notify immediately the police and
emergency crew do not engage in moving the person who are injured.
2. In case of caving in of excavated materials commence immediately excavation using
excavator if the person covered by sand is more than 1 meter deep in case of swallow
manual excavation shall be carried out.
3. In case of damage underground cables of infrastructure immediately distance yourself in
the area and notify the authorities and establish a perimeter barricade to avoid further
accident.
4. Emergency contact numbers including Police, Ambulance and Industrial Security will be
prominently posted on bulletin board.
5. First Aid box will be readily available on site.

3.7. Permit requirements

 The following are the required permit before implementing the activity:
1. Encroachment permit
2. Work permit
3. Transport Permit
4. Gate pass

4.1. Environmental Impact Assessment

 One possible risk to the environment is oil spillage may be due to equipment breakdown. If
and when this will happen immediately contained the area of contamination then remove
the contaminated soil. In order to avoid this incident proper maintenance procedure for the
equipment shall be implemented.
 Other possible risk to environment is garbage and debris accumulated thru-out the
construction activity, for this matter collection bin shall be used and proper disposal and
housekeeping shall be done.
 Other possible risk is water flow from channel or canal, should not be disturbed, when
disposing excavated soil, washing of vehicles and equipment.

5. QUALITY ASSURANCE

5.1. Contractual Requirements

 The quality assurance requirements base in our contract specification are the following:
1. Prior to the construction activity RC Engineer shall inspect the area to verify the area of the
repair.
2. RC Engineer shall inspect the approved materials which will be incorporated into works.
3. RC engineers shall inspect hold point release (HPR) and witness points as per ITP.

5.2. Critical Works

 The critical works will be in the compaction either the sub-base or backfilling as if the
watering and compaction procedure is not properly carried out it will surely fail in the
compaction test as all testing shall be witness by the RC Engineer. In addition to this in the
excavation proper benching and storing of excavated materials shall be implemented as
the RC Engineer will also check these activities.

5.3. Inspection & Testing

 When the excavation reaches the design elevation the sub-grade preparation
(Compaction) shall be implemented once its ready for Compaction Test(Field Density
Test) the Independent Testing Laboratory personnel shall conduct the testing witness by
the RC Engineer once it pass a Hold Point release will be signed by The RC Engineer will
also check these activities.
 The same thing with the backfilling once the layer were is completed and compacted a
compaction test (Field Density Test) shall be conducted by the Independent Testing
Laboratory personnel witness by the RC Engineer and the backfilling for the next layer
shall commence.

5.4. Request for Inspection (RFI)

 Request for Inspection of Work (RFI) shall be issued to RC Field Engineer 24 hours in
advance. RFI forms shall be issued.

5.5. Follow up & Evaluation

 We intend to follow up and evaluate our work by taking records of our accomplishment in
excavation or backfilling daily in order to evaluate if the existing equipment and manpower
is capable of completing the activity on time. In the event base from our daily records the
existing resources cannot complete in time additional resources shall be added to the
existing in order to accomplish it on time.
