find the equivalent fraction.
5.Study and take note on pages 4-5. In your Math notebook, write important details about the lesson on
how to Form Ratio and Proportion.

folder to serve as your PORTFOLIO or copy of your accomplishments as reference.

• Make a chart showing healthful habits that promote proper functioning of the body systems.

LEARNING TASKS:
1. Answer the "Pre-Test" and "Looking Back" on pages 2 - 3.
2. Read and understand the "Brief Introduction" about the lesson on page 4.

The test paper was distributed together with the module last Friday, January 15,2021. A soft copy is
posted here, but for those who cannot access the test online you can get a hard copy from the school
guard/utility on duty.

Learning Tasks:
1. Answer your test paper (hard copy) and let your parents/guardian bring it to school. Don't forget to
write your name and section.
2. Give the accomplished Summative Test to the guard/utility on duty to be placed in the retrieval box.
3. Please submit your Summative test until Thursday, January 21,2021.
