    Dear Maam:
    INCIDENT REPORT

    This is to inform your office that last October 26, 2020 one of our teacher has a close
    contact to the parents of the person suspected to have COVID 19. The teacher informed me
    very late that she was able to talk the mother of the suspected patent already in school. And all
    of us reported on the date except those who are working from home. The patient is under close
    monitoring with the Municipal Health Office and she was brought to isolation facility of Carmen
    last October 27, 2020. According to the family, the person was fetched by the Municipal Health
    Personnel to monitor the patient and to undergo swab test. On October 30, 2020, all family
    members of the Suspected COVID 19 patient were scheduled for swab test to check their health
    status. For us to be safe, though we are on 4 th line of contact I declare not to retrieve and
    release the modules on October 28-30, 2020 until such time the result will come out on
    November 4, 2020.

    This is to further inform that school is STRICTLY following health protocol. We also have
    the QR Code Application Scanner to check the coming in of our parents and visitors.
