A. Blood Specimen Extraction for CD4 Count Test to HIV PDL Patients in
Surigao del Norte District Jail
On 07 April 2021, JO1 Khimkem D Labucay, RN, Jail Nurse, conducted
Blood Specimen Extraction for CD4 Count to five (5) PDL infected with Human
Immunodeficiency Virus (HIV) and eighteen (18) suspected cases in Surigao City
Jail. Said specimen were then submitted to Caraga Regional Hospital for testing.

B. Temperature Monitoring of Personnel and PDL in Surigao City Jail

On 01-06 April 2021, JO1 Nathaniel Bonotan, RN, Jail Nurse, conducted
Temperature Monitoring to eighteen (18) Personnel on Lockdown (POL) and sixty
(60) PDL in Surigao City Jail.

C. Rapid Antigen Test in Bayugan City Jail

On 05 April 2021, SJO4 Isidro T Logronio and JO1 Junell C Simene
underwent Rapid Antigen Test at Bayugan City Health Office.

D. Physical Conditioning through Sunning Activity of PDL in Surigao del Norte
District Jail
On 03-05 April 2021, JO2 Rhea Laarni Lingatong, duty WDO, facilitated
the conduct of Physical Conditioning Activity through Sunning of one hundred fifty
(150) PDL in Surigao del Norte District Jail.

E. Triage Screening prior to Changing of Guards in Butuan City Jail Female

Dormitory
On 11 April 2021, SJO3 Girly Cariaga, RN and JO1 Carla Tosca Erika
V Guittap, RN, Triage Nurses, performed Triage and health assessment including
presence of influenza-like symptoms to five (5) Incoming Personnel on Lockdown
with Rapid Antibody Test (-) conducted by the Regional Health Service Division.

F. Disinfection and Decontamination of the Regional Office Premises

On 08 April 2021, the Health Service Division of BJMP Caraga led by SJO4
Richard C Recososa, RN, conducted Disinfection and Decontamination Procedure
to all contact surfaces of the BJMP Caraga Regional Office.

G. Swabbing Procedure for RT-PCR Testing of JO1 Trainees and Personnel in
BJMP Caraga Regional Office
On 07 April 2021, the Health Service Division of BJMP Caraga led by SJO4
Richard C Recososa, RN, conducted Swabbing Procedure to three (3) BJMP
Personnel and thirty-eight (38) JO1 Trainees due for turn-over to the Off-Campus
Training Site in Cagayan de Oro City.

H. Anti-Smoking Campaign in Butuan City Jail Female Dormitory

On 09-11 April 2021, JO1 Carla Tosca Erika V Guittap, RN, conducted Anti-
Smoking Campaign Information Drive to thirty (30) PDL and five (5) personnel on
lockdown of Butuan City Jail Female Dormitory.

I. Coordination of Surigao del Sur District Jail Tandag to Adela Serra Ty

Memorial Medical Center
On 12 April 2021, under the instruction of SJO4 Rosalito A Reyna,
Warden, JO2 Ermalyn Alas, conducted coordination efforts with Adela Serra Ty
Memorial Medical Center for the purpose of implementing the OPLAN ANDAM
COVID-19.

J. COVID-19 Vaccination of Surigao del Sur District Jail Tandag Personnel
On 12 April 2021, ten (10) personnel of Surigao del Sur District Jail
Tandag received the first dose of Sinovac COVID-19 Vaccine in the vaccination
center of Tandag City, Surigao del Sur, through the Local Government Unit of
Tandag City.

K. Distribution of Face Masks in Bayugan City

On 03 April 2021, JO1 Jennifer S Jamero, RN, Jail Nurse, distributed
face masks to forty-two (42) PDL in Bayugan City Jail. Each PDL received one (1)
face mask.

L. Health Monitoring of PDL and Personnel in Butuan City Jail Female

Dormitory
On 02, 09 and 11 April 2021, JO1 Carla Tosca Erika V Guittap, RN, Jail
Nurse, conducted Health Monitoring for PDL and Personnel in Butuan City Jail
Female Dormitory whereby six (6) personnel and thirty (30) PDL were able to
benefit.

M. Temperature Monitoring of Personnel and PDL in Surigao City Jail
On 07-12 April 2021, JO1 Nathaniel Bonotan, RN, Jail Nurse, conducted
Temperature Monitoring to eighteen (18) Personnel on Lockdown (POL) and sixty
(30) PDL in Surigao City Jail.

N. 5PM Habit Kontra COVID-19 in Butuan City Jail Female Dormitory

On 08-10 April 2021, JO1 Carla Tosca Erika V Guittap, RN, Jail Nurse,
implemented “5 o’clock Habit Kontra COVID-19” program in Butuan City Jail
Female Dormitory, as part of the STOP COVID-19 INITIATIVE PROGRAM,
through clean up and disinfection of all contact surfaces of the sail jail facility.

O. Vaccination of Personnel in Surigao del Sur District Jail Annex Lianga

On 14 April 2021, twenty-one (21) SDSDJA-LIANGA JNOR personnel
were successfully inoculated with the first dose of Sinovac vaccine against
coronavirus disease (Covid-19) in the Department of Health Vaccination hub
located at Surigao del Sur State University – Lianga Campus, Lianga, Surigao del
Sur.

P. 4 PM Habit Para Deng-get Out Program in Butuan City Jail Female Dormitory
On 11 April 2021, JO1 Carla Tosca Erika V Guittap, conducted the 4PM
Habit para-Deng-get Out in Butuan City Jail Female Dormitory through clean up
and eradication of all mosquito breeding sites within the jail facility.

Q. Triage Screening prior to Changing of Guards in Butuan City Jail Female

Dormitory
On 10 April 2021, JO1 Carla Tosca Erika Guittap, RN, Jail Nurse,
conducted feeding program to all forty-eight (48) PDL of Butuan City Jail Female
Dormitory.

R. Information Drive on How to Manage Stress at Work

On 05 April 2021, JO1 Jennifer S Jamero, RN, Jail Nurse, conducted
information drive on the topic of How to Manage Stress at Work to seven (7)
lockdown personnel of Bayugan City Jail.

S. Psychosocial Activity in Butuan City Jail Female Dormitory
On 10 and 11 April 2021, JO1 Carla Tosca Erika Guittap, RN, Jail Nurse,
conducted a Psychosocial Activity to a total of twenty (20) PDL in Butuan City Jail
Female Dormitory.

T. Rapid Antibody Test of Incoming Lockdown Personnel

On 10 April 2021, thirteen (13) incoming lockdown personnel of Surigao
del Norte District Jail underwent Rapid Antibody Testing for COVID-19 Virus in
Surigao del Norte Provincial Hospital in Sta. Cruz, Placer, Surigao del Norte.

U. Temperature Monitoring of PDL in Butuan City Jail Female Dormitory

On 14 April 2021, JO1 Carla Tosca Erika V Guittap, RN, conducted
temperature monitoring to forty-eight (48) PDL in Butuan City Jail Female
Dormitory.

V. Triage Screening of Two (2) Incoming Personnel in Bayugan City Jail
On 05 April 2021, JO1 Jennifer S Jamero, RN, Jail Nurse, conducted
Triage Screening to two (2) Incoming Personnel on Lockdown (POL) in Bayugan
City Jail Triage Area.

W. NVOC- BJMP-BUCOR-DSWD-DILG Meeting confirmation for PDL

Vaccination
On April 16, 2021, JSSUPT JOLLY C TAGUIAM JR, Regional Director,
SJO4 Richard C Recososa, Regional Health Division Chief, Unit Wardens, and
Unit Nurses under BJMP Caraga region attended the Orientation for the
Vaccination Roll-out to PDL's and Bedridden Individuals Under Priority A2 and A3,
conducted by the Department of Health through Zoom platform.

X. 5PM Habit Kontra COVID-19
On 10-20 April 2021, the 5PM Habit Kontra COVID-19 program is being
continuously implemented in all twelve (12) jail units in BJMP Caraga through the
conduct of clean up and disinfection activity of all contact surfaces of jail units.

Y. Anti-Smoking Campaign in Surigao del Norte District Jail

On 10 April 2021, JO1 Glenford Forcadilla, RN, Jail Nurse, conducted
Anti-Smoking Campaign through information drive to twenty (20) PDL in Surigao
del Norte District Jail.

Z. Sunning of Bedclothes in Butuan City Jail Female Dormitory

On 10 April 2021, JO1 Carla Tosca Erika Guittap, RN, Jail Nurse
facilitated the sunning of bedclothes of PDL in Butuan City Jail Female Dormitory.

AA. Hospital Admission of one (1) PDL in Lianga District Hospital
On 11 April 2021, JO3 Roger Maceda escorted one (1) PDL for
admission to Lianga District Hospital after due assessment and intervention
rendered by JO1 Army Jaictin, RN, Jail Nurse. Said PDL was found by JO1 Jaictin
to be experiencing malaise, loss of appetite and on/off fever for three (3) days.

BB. Physical Conditioning through Sunning and Morning Exercise

On 10-15 April 2021, JO1 Graciella Marie Ebro, Jail Nurse, facilitated the
conduct of Physical Conditioning Activity through Sunning of fifty (50) PDL in
Cabadbaran City District Jail.

CC. Physical Conditioning of PDL through Sunning in Surigao del Norte

District Jail
On 08-15 April 2021, JO2 Rhea Laarni Lingatong, duty WDO, facilitated
the conduct of Physical Conditioning Activity through Sunning of two hundred (200)
PDL in Surigao del Norte District Jail.

DD. Triage Screening prior to COG in Butuan City Jail Male Dormitory
On 11 April 2021, SJO3 Florefe M Badillo, RN, Jail Nurse, conducted
Triage Screening to seventeen (17) incoming lockdown personnel of Butuan City
Jail Male Dormitory, after they tested negative (-) for Rapid Antigen Test conducted
by JO1 Jessie Z Doysabas through the Regional Diagnostic Laboratory.

EE. Triage Screening prior to COG in Cantilan District Jail

On 12 April 2021, JO2 Diana Christy O Quinonez, RN, Jail Nurse,
conducted triage screening to six (6) incoming lockdown personnel of Cantilan
District Jail after they were tested (-) negative for rapid antibody test.

FF. Triage Screening prior to COG in Surigao del Norte District Jail
On 12 April 2021, JO1 Kristy Lou Pacillos, RN, Jail Nurse, conducted
triage screening to ten (10) incoming lockdown personnel of Surigao del Norte
District Jail after they were tested (-) negative for rapid antibody test.

GG. Physical Conditioning of Personnel through Zumba Exercise in
Surigao del Sur District Jail Tandag
On 12 April 2021, SJO4 Rosalito A Reyna, OIC-Warden, enjoined the
conduct of Zumba Exercise to nine (9) lockdown personnel of Surigao del Sur
District Jail Lianga.

HH. Virtual Health Service Provider’s Meeting

On 21 April 2021, under the leadership of J/SSUPT JOLLY C TAGUIAM
JR, Regional Director, SJO4 Richard C Recososa, RN, Chief, Health Service
Division, conducted a Virtual Meeting for all the Health Service Providers of BJMP
Caraga through Google Meet online platform. During the meeting, twenty-six (26)
health practitioners all over the region were able to attend and participate the said
activity.

II. Coordination on the Conduct of Mobile Vaccination for COVID-19
Vaccination of Bayugan City Jail PDL
On 16 April 2021, JO1 Angelita N Aranton, response team member,
undertook coordination efforts to the City Health Office of Bayugan for the conduct
of mobile vaccination for Bayugan City Jail PDL classified as senior citizens and/or
with co-morbidities.

JJ. COVID-19 Vaccination of Butuan City Jail Male and Female Dormitories
Personnel
On 15 April 2021, six (6) personnel from Butuan City Jail Male Dormitory
and four (4) personnel of Butuan City Female Dormitory were successfully
inoculated with the first dose of Sinovac COVID-19 vaccine against coronavirus
disease (COVID-19) in the Department of Health Vaccination hub located at Father
Saturnino Urios University Gymnasium, Butuan City.

KK. COVID-19 Vaccination of Cantilan District Jail Personnel

On 14 and 15 April 2021, respectively, sixteen (16) personnel of Cantilan
District Jail were successfully inoculated with the first dose of Sinovac COVID-19
vaccine against coronavirus disease (COVID-19) in the Department of Health
Vaccination hub located at Cantilan Gymnasium, Cantilan, Surigao del Sur.

LL. COVID-19 Vaccination of Cabadbaran City District Jail Personnel
On 15 April 2021, seven (7) personnel of Cabadbaran City District Jail
were successfully inoculated with the first dose of Sinovac vaccine against
coronavirus disease (COVID-19) in the Department of Health Vaccination hub
located at Cabadbaran City Gymnasium, Cabadbaran City.

MM. COVID-19 Vaccination of Surigao del Norte District Jail Dapa Annex
Personnel
On 13 and 15 April 2021, respectively, eight (8) personnel of Surigao del
Norte District Jail Annex Dapa were successfully inoculated with the first dose of
Sinovac vaccine against coronavirus disease (COVID-19) in the Department of
Health Vaccination hub located at Dapa Municipal Covered Court.

NN. COVID-19 Vaccination of Regional Office, Butuan City Jail Male and
Female Dormitories Personnel
On 05 April 2021, fifteen (15) personnel of BJMP Regional Office, nine (9)
from Butuan City Jail Male Dormitory and seven (7) personnel of Butuan City
Female Dormitory were successfully inoculated with the first dose of AstraZeneca
COVID-19 vaccine against coronavirus disease (COVID-19) in the Department of
Health Vaccination hub located at Father Saturnino Urios University Gymnasium,
Butuan City. In sum, thirty-one (31) BJMP personnel were vaccinated in this
activity.

OO. COVID-19 Vaccination of Surigao City Jail Personnel
On 12 and 13 April 2021, respectively, fifty (50) personnel of Surigao
City Jail were successfully inoculated with the first dose of Sinovac vaccine against
coronavirus disease (COVID-19) in the Department of Health Vaccination hub
located at Surigao City Auditorium, Surigao City.

PP. Rapid Antigen Test of Incoming Lockdown Personnel in Bayugan City
Jail
On 19 April 2021, seven (7) incoming lockdown personnel of Bayugan
City jail underwent Rapid Antigen Test in the City Health Office of Bayugan. All
personnel were tested negative for said test.

QQ. Rapid Antigen Test of PDL in Butuan City Jail Male Dormitory
On 15 April 2021, JO1 Jessie Z Doysabas, RMT, Regional Medical
Technologist, conducted Rapid Antigen Tests to five (5) Insular PDL due for
transfer to Davao Penal Colony and one (1) PDL due for transfer to Drug
Rehabilitation Center- Anomar, Surigao City. The said undertaking was done at
BCJ-MD Compound, Libertad, Butuan City.

RR. Rapid Antigen Test of Insular Prisoners in Cantilan District Jail

On 15 April 2021, SJO2 Bonifacio G Sanchez, Jr, SJO1 Myleen P Lupas
and JO3 John Erwin T Azarcon escorted three (3) insular prisoners to the Madrid
Municipality Rural Health Unit for the conduct of rapid antigen test. The three (3)
PDL yielded negative results.

SS. Contact tracing and Conduct of RT-PCR testing of 10 (ten) COVID19
direct contact personnel in BJMP Caraga region
As part of the contact tracing and COVID19 infection prevention and control
efforts of the BJMP Caraga, 9 (nine) BJMP Caraga Regional Office personnel and
1 (one) staff, all asymptomatic but who were in direct contact of COVID19-positive
personnel, underwent RT-PCR testing conducted by the Health Division staff
headed by SJO4 Richard C Recososa, last April 14, 2021 at the Regional Office
swab booth. The Swab specimens taken were then submitted to the Butuan
Medical Center Molecular Laboratory for RT-PCR Testing.

TT. 5 PM Habit Kontra COVID-19
On 21-22 April 2021, the 5PM Habit Kontra COVID-19 program is being
continuously implemented in all twelve (12) jail units in BJMP Caraga through the
conduct of clean up and disinfection activity of all contact surfaces of jail units.

UU. Consultation and Health Monitoring of PDL in Surigao del Norte

District Jail Dapa
On 19-20 April 2021, JO1 Jovanie Guiral, RN and JO1 Jhay-Ar Dayo,
RN, jail nurses, conducted Consultation and Health Monitoring to twenty (20) PDL
in Surigao del Norte District Jail Dapa.

VV. Disinfection of Swab Booth in Butuan City Jail Male Dormitory

On 15 April 2021, JO1 Alison Aikko U Asis, RN, Jail Nurse, facilitated
the conduct of disinfection of the swab booth of Butuan City Jail Male Dormitory.

WW. Drug Awareness Information Drive in Butuan City Jail Female
Dormitory
On 10 April 2021, JO1 Carla Tosca Erika V Guittap, RN, Jail Nurse,
conducted an Information Drive on Drug Awareness Prevention to sixteen (16)
PDL of Butuan City Jail Female Dormitory.

XX. Drug Test and Drug Dependency Examination of Two (2) PDL in
Surigao del Norte District Jail
On 16 February 2021, under the direction of JCI JOEL G CALIMBO,
Warden, JO1 Rea Sheen Hinampas, RN, Jail Nurse, conducted a Drug Test of two
(2) PDL in Surigao del Norte District Jail. Subsequently, the PDL was subjected to
a Drug Dependency Examination (DDE) scheduled on the same date, conducted
by Dr. Victoria Labonete.

YY. Physical Conditioning Activities for PDL through Morning Exercise in

Butuan City Jail Male Dormitory
On 12-16 April 2021, respectively, JSI DIOVIN C AUZA, Warden, enjoined
the conduct of Physical Conditioning activities for PDL through Morning in the
basketball court of Surigao del Norte District Jail. A total of five hundred (500) PDL
were able to benefit from this activity.

ZZ. Rapid Antigen Test of Incoming Lockdown Personnel in Bislig City Jail and
Bislig District Jail
On 12 April 2021, nine (9) incoming lockdown personnel of Bislig City Jail
and Bislig District Jail underwent Rapid Antigen Test in the City Health Office of
Bislig. All personnel were tested negative for said test.

AAA. Rapid Antigen Test and Triage Screening of Incoming Lockdown

Personnel in Surigao City Jail
On 13 April 2021, JO1 Jessie Z Doysabas, RMT, Regional Medical
Technologist, conducted Rapid Antigen Test to eighteen (18) incoming lockdown
personnel of Surigao City Jail.

BBB. Temperature Monitoring in Surigao City Jail

On 12-22 April 2021, SJO2 Laurence Ortega and JO1 Nathaniel Simon
Bonotan, respectively, conducted Temperature Monitoring to eighteen (18)
lockdown personnel and four hundred three (403) PDL in Surigao City Jail.

CCC. Application of Wound Dressing in Butuan City Jail Male Dormitory
On 16 April 2021, JO1 Alison Aikko U Asis, RN, conducted application
of wound dressing to one (1) PDL who underwent Sebaceous Cyst Removal. This
was conducted at the TB-DOTS facility of Butuan City Jail Male Dormitory.

DDD. Triage Screening Prior to Changing of Guards in Lianga District Jail

On 14 April 2021, JO1 Army P Jaictin, RN, Triage Nurse successfully
conducted Triage Assessment to seven (7) incoming lockdown personnel of
Lianga District Jail.

EEE. Consultation and Health Monitoring in Bayugan City Jail

On 13 April 2021, JO1 Jennifer S Jamero, RN, Duty Nurse, conducted
Consultation and Health Monitoring Activity to thirteen (13) PDL in Bayugan City
Jail.

On 13 April 2021, SJO2 Rhodora Gamale, RN, Duty Nurse, conducted

GGG. Medical Check-Up and Minor Operation of PDL of Butuan City Jail
Dormitory
On 13 April 2021, JO1 Jerry Casawitan and JO1 Rico R Calo, escort
personnel, properly escorted one (1) PDL of Butuan City Jail Male Dormitory to
Agusan del Norte Provincial Hospital through BJMP Prisoner’s Van for his Medical
Check-up and Minor Operation for Removal of Sebaceous Cyst, Right Ulna.

HHH. Medical Check-Up of five (5) PDL of Butuan City Jail Male Dormitory
On 13 April 2021, JO1 Jerry Casawitan and JO1 Rico R Calo, escort
personnel, properly escorted five (5) PDL of Butuan City Jail Male Dormitory to Butuan
Medical Center through BJMP Prisoner’s Van for their Medical Check-up and
Laboratory Examination, as pre-requisite for their transfer to Davao Penal Colony.

On 18 April 2021, JO2 Junie Naungayan, RN, Duty Nurse and JO3
Roger Maceda, escort personnel, referred and admitted one (1) PDL of Lianga
District Jail in Adela Serra Ty Memorial Medical Center.

JJJ. Information Drive on Summer Diseases

On 17 April 2021, JO1 Ron Joshua L Mante, Duty Nurse, conducted an
information drive on summer diseases such as sore eyes, flu, scabies, dog & cat
bites, sunburn and diarrhea.

KKK. Information Drive on Summer Diseases in Cabadbaran City District

Jail
On 21 April 2021, JO1 Graciella Marie S Ebro, RN, Duty Nurse
successfully conducted Information Drive on Summer Diseases in Cabadbaran
City District Jail. This was attended to and participated by ten (10) PDL.

Tandag
On 14 April 2021, SJO2 Rhodora S Gamale, RN, Duty Nurse
successfully conducted Information Drive on Summer Diseases in Cabadbaran
City District Jail. This was attended to and participated by thirty (30) PDL.

MMM. 5 O’clock Habit Kontra COVID-19 in Surigao del Sur District Jail
On 1700H 26 April 2021, under the supervision of SJO4 Rosalito Reyna,
duty personnel facilitated and supervised one (1) male jail aide in facility
disinfection as a support to the program launched by the Regional Director
JSSUPT JOLLY CASTIGO TAGUIAM, JR re “Daily 5 O’clock Habit Kontra COVID-
19”, as a mandatory daily clean-up and disinfection in all BJMP Jail facilities
throughout the region.

NNN. Annual Medical Examination in Bayugan City Jail
OOA on 0800H-1030H 26 April 2021, under the leadership of JSINSP
ANDRES T ESPINOZA JR, JO1 Graciella Marie S Ebro, RN, Duty Nurse
facilitated the health personnel from Butuan Diagnostic and Medical Clinic who
arrived in the unit to conduct medical examination. Said health personnel
successfully conducted series of medical examination to all (26) twenty-six
personnel.

OOO. Annual Medical Examination in Bayugan City Jail

OOA 24 0700H April 2021, health personnel from Butuan Diagnostic and
Medical Clinic arrived in the unit and successfully conducted series of medical
examination to all (18) eighteen personnel. Also, personnel disinfect the said
mobile before proceeding to X-ray, ECG and specimen collection for the said
Medical Examination.

PPP. Annual Medical Examination in Bislig City Jail and Bislig District Jail
OOA 24 0700H April 2021, health personnel from Butuan Diagnostic and
Medical Clinic arrived in the unit and successfully conducted series of medical

On 26 April 2021, JO1 Alvin Blasquez, Duty IWDO, secured by SJO1
Willie Requillo Shift In-charge, together with the rest of the lockdown
team personnel, under the supervision of JCINSP JOEL CALIMBO, temporarily
out celled and supervised the sunning activity of the ninety (90) PsDL from Dorm
4-7 to pre-occupy them in the morning.

RRR. Surprised Jail inspection and Drug Testing of Personnel and PDL in
Butuan City Jail
On 29 April 2021, some (150) one hundred fifty randomly selected Persons
Deprived of Liberty which represent the 20% of the overall PDL population and
(41) forty-one personnel including the Butuan City Jail Male Dorm Warden JSINSP
DIOVIN T AUZA underwent the drug testing procedure conducted by BJMP
Caraga Health Service Division led by SJO4 Richard C Recososa.

SSS. Surprised Jail inspection and Drug Testing of Personnel and PDL in
Cabadbaran City District Jail
On April 22, 2021, BJMP Caraga headed by J/SSUPT JOLLY C TAGUIAM
JR. together with the BJMP Caraga Operations Division, Chief JCINSP MARGIE
CANANGCAAN, and through Health Service Division led by SJO4 Richard C
Recososa conducted unannounced jail inspection and drug testing of twenty-six
(26) BJMP personnel including JSINSP ANDRES T ESPINOZA JR, OIC Warden,
and thirty-four (34) PDL or 20% of the total Jail Population in Cabadbaran City
District Jail.

TTT. Consultation and Health Monitoring in Surigao City Jail
On 0755H-0810H 26 April 2021, SJO2 Laurence Ortega, Duty Nurse of
Surigao City Jail, under the supervision of JCINSP ROBERTO C GOTICO,
conducted the following Health Assessment in the unit’s quarantine cell where four
(4) PDL and four (4) personnel were benefited

UUU. Medical Check-up of (1) One PDL

On 0830H-1100H 27 April 2021, with the supervision of JSINSP DIOVIN
AUZA, (1) one PDL Asraf Ampaso, 25 years old, was brought to Agusan del Norte
Provincial Hospital, Butuan City, for medical check-up & laboratory examination,
due to intermittent headache, dizziness & loss of appetite.

VVV. Conduct of Temperature Monitoring of BJMP Lockdown Personnel
and JO1 Trainees
On 0840H 26 April 2021, under the supervision of SJO4 Richard C
Recososa, JO1 Mardenver R Balan alternate duty health, conducted body
temperature monitoring of BJMP lockdown personnel and JO1 Trainees to detect
any signs of COVID-19 infections and other flu like symptoms.

OOA 0600H 27 April 2021, All (52) fifty-two Personnel of the BJMP Caraga
Regional office, led by the very active Regional Director JSSUPT JOLLY C
TAGUIAM JR, underwent a free Annual Medical and Diagnostic Examination
package at the premises of the BJMP Caraga Regional Office courtesy of the
Butuan Diagnostics and Medical Clinic through its Mobile-based clinic.

XXX. 5 PM Habit Kontra COVID-19
On 23-30 April 2021, the 5PM Habit Kontra COVID-19 program is being
continuously implemented in all twelve (12) jail units in BJMP Caraga through the
conduct of clean up and disinfection activity of all contact surfaces of jail units.

YYY. Mobile Annual Diagnostic Medical Examination Specimen Collection

in Surigao City Jail
On 24 April 2021, the Mobile Diagnostic Clinic of the Butuan Diagnostic and
Medical Clinic conducted specimen collection for the Annual Diagnostic Medical
Examination of fifty (50) BJMP personnel of Surigao City Jail. SJO2 Laurence
Ortega, RN, SJO2 Lynn M Cuadrillero, RN and JO1 Joar Dedumo, RN assisted
during this activity.

ZZZ. Mobile Annual Diagnostic Medical Examination Specimen Collection
in Surigao del Norte District Jail
On 24 April 2021, the Mobile Diagnostic Clinic of the Butuan Diagnostic
and Medical Clinic conducted specimen collection for the Annual Diagnostic
Medical Examination of forty (40) BJMP personnel of Surigao del Norte District
Jail.

AAAA. Belly Gud Program in Surigao del Norte District Jail Dapa Annex
On 26 April 2021, JCI RAUL C BADILLO, Warden, enjoined the conduct of
the Belly Gud Program to eight (8) Personnel of Lockdown in Surigao del Norte
District Jail Dapa Annex. This was done through abdominal girth taking, weight
taking and exercise.

BBBB. Rapid Antibody Testing of Incoming Personnel in Butuan City Jail

On 27 April 2021, one (1) incoming Personnel on Lockdown of Surigao del
Sur District Jail Tandag named JO1 Kim Tonera underwent Rapid Antibody
Testing prior to his assumption of duties in the said jail unit.

On 22 1600H February 2021, JO2 Fritz Anthony C Salazar, Regional
Psychometrician, conducted Telepsychology: Psychosocial Processing (PSP) to
one personnel of Regional Office Caraga who confirmed positive for COVID-19.

EEEE. NP Evaluation to JO1 Applicants
On 15-24 April 2021, SINSP DONNABELLE G QUINDIPAN, MD, Regional
Psychiatrist for Visayas-Mindanao, conducted the Neuro-Psychiatric Evaluation to
Two hundred forty-three (243) JO1 applicants of BJMP-Caraga.
