In the FWD Max Apps, there is an E-Friend as below:

E-friend is another new feature provided by FWD Life to help


Click e-friend on customers when checking claims, monitor claims quickly and easily.
FWD MAX Apps
If the customer wants to submit an e-Friend reimbursement they
only need to upload all the required claim documents.

There are option menu in

PolicyInformation
Policy Information contains information on your details (plans and
benefits)..

How to see Policy Information as below:


1. Click the Policy Information button
2. Click your member
3. Click Policy Year
4. Click your program policy
5. Click Button Search

MemberInformation
Member Information contains your personal data information, benefits How to see member information as below:
and you can also update your data here. 1. You click the member Information button

How to Edit a Profile as below:
1. You click the Edit profile button, and you directly edit the data that you
want to change. For example: Mobile Number
2. Don't forget to click the Save Profile button

E-Claim Submission
E-Claim Submission is a menu to submit claims according to your eligible benefits

Functioning for you to submit a reimbursement claim to FWD:


1. Click the Reimburse Claims Button,
2. Input date of treatment/visit date and discharged date,
3. Choose name (member name), Benefit Type, Hospital/Clinic Name, Fill in the nominal claim amount
(total amount), doctor name, Click “Next”
4. Prepare all claims documents,
5. Take a picture for original receipt/invoice, please write a note “already claimed to FWD” above the
receipt/invoice before being photographed,, Click “Next”
6. Upload all your medical documents, Click “Next”
8. Fill your detail bank account for payment,
9. If it is correct, Click “Submit Reimbursement Claim”
10. Your e-Claim have been submitted successfully. Click “Tracking Progress” for tracing your claims status.
11. Give your rate and feedback

ClaimAccumulation
Claim Accumulation is your accumulated claims that record in this menu.
How to see your Claim Accumulation as below:
1. You click the Claim Accumulation button
2. Click your policy, member number, benefit & Policy Year
3. Click the Search button, all claim accumulation will appear in this menu.

ExcessClaim
Excess Claim is your excess claim menu. If you have excess it will appear all in this menu.

How to view your Excess Claim as below:


1. You click the Excess Claim button
2. All displays of excess will appear on this page if you have
3. excess claim.

Claim information is contains your all claim information.
How to see the How To Guide as below:
1. You click the button Claim information
2. Click your policy, member number and don't forget to click Search
3. Choose the No Claim you will see

Provider Map is contains of FWD ‘s Provider list

How to view the Provider Map as below:

REIMBURSEMENT
CLAIM PROCEDURE HALODOC
REIMBURSEMENT CLAIM PROCEDURE HALODOC

✓ Participants can submit reimbursement claims for treatment/consultation/purchase of prescribed


medication through the Halodoc application.
✓ Participants submit document claims within 90 days of discharge from date of
treatment/consultation/purchase of prescribed medication (including re-claim).

• Documents to completed as follows:


✓ Original Receipt/invoice in Halodoc menu (capture/scan copy).
✓ "capture chat / consultation" with the doctor, which indicates the doctor's recommendation on the
drug/treatment given to the participant. The document is attached only to the end of the chat,
where there is information Notes form doctor for medical indication and diagnosis information.
✓ Please ensure when completing the data in Halodoc, participant name must be in accordance with
the name registered in the FWD to avoid obstacles during claim process.
REIMBURSEMENT CLAIM PROCEDURE HALODOC

• Claims that have been received will be processed :

✓ If through the e-Friend portal will be processed within 5 working days since the
original claim document received by FWD Life.

✓ If not through e-Friend portal, it will be processed within 7 - 10 working days since
the original claim document received by FWD Life.

• Documents that do not meet the above requirements will be informed by email to
Participants to be completed within 30 days from the notification sent by FWD Life.
