I am fully aware that such item is in very good condition and is part of the 2 in 1 TABLET PC which was
delivered from DepEd Central Office and received by Mabolo National High School last September 3,
2019.

I am also fully aware that, if granted approval, the aforementioned item will be used for Educational
Purposes only, specifically for the Online Enrollment and Online Distance Learning as one of the
Learning Delivery Modalities which will be utilized in the new normal in education and not for purposes
other than those for which it was allowed to be borrowed.

If such request is approved, I will accept the 2-in-1 Tablet PC and hold myself responsible for its
acceptance. It is to my full knowledge that in case of loss and/or damage caused by mishandling and
negligence, I am accountable for the said loss and/or damage and am willing to replace the borrowed
item or pay the required amount.
