1.0 INTRODUCTION

This Standard has been prepared to meet the conditions imposed by local Egyptian regulations and
LUFKIN Middle East Standards to safeguard Lufkin employees and persons when working in onfined
space.
In our risk assessment we observed no hazards related to confined space due to the limited tasks
associated with this area, but this document is issued to protect Lufkin people even in case we exposed
to 1% of this risk to achieve Zero incident goal.
This document is to establish the minimum requirements to assure that any person entering a confined
space will not be subjected to conditions that may cause "immediate Danger to life and health" To be
defined a confined space is an area that must meet all of the following criteria:

 Be large enough to permit an employee to bodily enter to work


 Have limited and restricted means for entry or exit.
 Is not equipped or designed for continuous employee occupancy.
 If one or more of the criteria are not met, the confined space
requirements do not apply.

The purpose of this Standard is to provide practical guidance to all individuals and/or
Contractors, working on, or supervising work on, electrical equipment and systems within
LUFKIN MIDDLE EAST site such that all work will be carried out safely, injury will be
prevented, and there will be compliance with the appropriate statutory national legislation.

2.0 SCOPE

This procedure applies to any Lufkin employees, contractors required to enter a confined
space. Lufkin Middle East has only class "B" confined space that normally does require entry
permit. Any permit for entering confined spaces shall be coordinated with the customer safety
manager who will provide us with all protection and supervision means.

3.0 Responsibilities:
1. Lufkin Middle East shall enforce the CS procedures for entry into CS.EHS manager will
decide on disciplinary actions on the advice of incident investigation plan and team.
2. The location supervisor has primary responsibility for preparing the CS for entry, and for
verifying that the conditions are suitable prior to entry.
3. Any questions regarding CS entry program or CS hazards can be addressed to EHS
manager.
4. The confined space entry program will be reviewed and discussed during the weekly
and monthly meeting with the operation manager.

Duties of entrant:

Being aware of potential hazards both inside and outside the confined space.
Wearing the required PPE when entering the Confined space.
Complying with all safety rules.

Following the CS Permit requirements.

Duties of attendant:

Has the authority to order the evacuation of the confined space.


Following the CS permit requirements.

Duties of entry supervisor:

Verifying the appropriate tests, entries, procedures, equipments & rescue services are in place prior
to authorizing entry to the confined space.
Conducting a pre- entry briefing with all involved persons
Signing an entry permit.

4.0 GENERAL PRINCIPLES AND PROCEDURES FOR ENTERING A CONFINED SPACE:


LUFKIN Middle East Lockout / Tagout and try procedures are followed to isolate energy
sources including auxiliary equipment.
Entry will be permitted only after the entrants read and understand the procedures and sign of
Cs.

4.1 ventilation

Air movement shall be provided through the confined space to maintain the oxygen
concentration at normally (21%).
Oxygen concentration below 19.5% and above 23.5 % will require exit from the
confined space.
Sufficient vents, mechanical air movers shall be opened/ utilized to assist in providing
fresh air in the confined space.
Flammability: should be less than 10 % of LEL
Carbon monoxide should be less than 25 ppm
Hydrogen sulphide less than 10 ppm

4.2 Testing

Air sampling and monitoring instrument will be calibrated once a year and calibration
certificate will be kept for a year. Completing air sampling and monitoring records will be
kept indefinitely, prior to initial entry, after each interruption and when deemed
necessary during the entire job sequence. Testing shall; be done for oxygen level, by
the location supervisor.

3.3 rescue:

Will be performed by fire brigade existing in the location. The location supervisor will
alert the fire brigade telephonically prior to confined space entry.

3.4 Training"
It is the responsibility of the site management to organize and perform periodic
training to all employees who are concerning in working within the confined space.
The frequency of training is at least once a year.

He should send a request of training requirements to HR department and EHS manager


and consequently the training program will be organized and planned in a manner that
will not violate with working in the field.

3.5 Record keeping

Air sampling instrument calibration records will be maintained for a period of one. If any
permit required confined space is entered the records will be maintained for a period of
one year. The air monitoring records during an entry will be maintained indefinitely.

5. CONFINED SPACE ENTERY PROCEDURE

Because of the low likelihood of hazards associated with class '" B" confined space
areas, procedures for entry has special provisions.

CAUTION :DO NOT ASSUME THE SAFETY OF ANY CONFINED SPACE

5.1 Preparation for entry in to confined space

A safe work instruction (SWI) is required for each entry in to confined space. All entrants
will follow the SWI carefully. This may require a step by step analysis of safety
procedure for the specific job being done.

B. Confined space entry checklist

Confined entry checklist has to be filled out and signed by the entrant(s), observer and
authorizing location supervisor.

C. Confined space entry permit:

Location supervisor after informing the EHS manager and the operation manager must
prepare a confined space entry permit in case of the need to perform work inside the
confined space.

D. Special work practices

No entry will take place unless permission is granted by the location supervisor and the
entry program is read and understood.

E. RESCUE PLAN FOR PERSONNEL ENTERING CONFINED SPACE:

An essential requirement for every confined space entry is the presence of an attendant
who will effectively and safely remove individuals from the enclosure in the event the
unexpected happens.

F. Attendant:

One person should be stationed outside the confined space when internal work is being
performed maintaining continual contaminations with the individual(s) in the enclosure. This is
the primary duty of the attendant. The attendant should be alert for changing conditions and
how to summon assistance immediately. He is expected to take all emergency actions short of
entering the confined space until emergency assistance arrive
