ACCIDENTS

 Each year a number of people are killed and lots are


injured due to lifting accidents
 The vast majority of accidents are not due to faulty
lifting equipment
 The majority of accidents are due to misuse of lifting
equipment i.e. unsafe practises

 A person is an individual or a corporate body


 Lifting equipment is equipment for lifting or lowering
loads and includes attachments used for anchoring,
fixing or supporting it.
 Thorough examination means examination by a
competent person and includes testing if appropriate
 Load includes a person
 “Accessory for lifting” means work equipment used to
attach a load to a lifting device

Regulation 4.
Strength & Stability

 Lifting equip. Must be of adequate strength and stability for


each load having regard in particular to the stress induced at its
mounting or fixing point
 Every part of a load and anything attached to it and used in
lifting it must be of adequate strength (timber pallets for
instance)
 For stability consider mounting surfaces, overturning moments,
mobility, counterbalancing, outriggers, anchorage etc)

Regulation 5.
If lifting persons

 Eliminate risk of person being crushed, falling,


getting trapped or being struck.
Or else
 Enhanced safety coefficient for suspension
rope or chain ( X 2 )
and
 Inspection each working day by a Competent
Person.

Positioning & Installation

 Risk of lifting equipment or a load striking a person to


be as low as reasonably practicable
 Risk from a load drifting, falling freely or being
released unintentionally must be as low as reasonably
practicable (Including power failure)
 Suitable devices to prevent a person from falling down
a shaft or hoist way
 Minimise risk of collision

Marking

 Machinery & accessories marked with S.W.L. including marking for


different configurations (e.g. lifting beams, telescopic reach trucks)
 Accessories marked to show characteristics necessary for safe use. (or
clear instructions)
 If for lifting persons should be marked to say so (and number of
persons)
 If not for lifting persons but could be so used in error should carry a
warning

Organisation of Lifting Operations

 Must be planned by a competent person

 Must be appropriately supervised

 Must be carried out in a safe manner

Case Study

 Jack Tangney, a 29 year old died when a large timber panel, fell
to the ground whilst it was being lifted to the 9th floor
 Investigating inspector Emma Davies said;
"The risk of serious injury, or worse from badly planned lifting operations
is well known; as are the measures needed to manage them. This
avoidable incident is an example of how badly things can go wrong when
lifting operations are not planned or supervised properly. When carrying
out lifting operations on site, three key elements must be in place to
ensure the safety of the lift
 “a competent person is involved in planning the lift”
 “the lift is adequately supervised”
 “and the lift is carried out in a safe manner”

Regulation 9.
Thorough Examination & Inspection

 Before equipment is put into service unless it is new and has an EC


Declaration of Conformity.
 If safety depends on installation, before equipment is used and if moved
to a new site or installation.
 Equipment for lifting persons & accessories at least every six months.
 If exposed to conditions causing deterioration.
 Other equipment at least every 12 months .
 Or in accordance with “a scheme”.
 In exceptional circumstances.
 Inspection (if appropriate) At suitable intervals between examinations.

Examiner’s Reports & Records

Regulation 11.
Examination reports must be kept: -

 In any/all cases
 Where a defect is reported until the next examination report is made

 In the case of
 Examination before putting into service and with the exception of
accessories for lifting, until the employer ceases to use the equipment.
 Examination before putting into service of a lifting accessory, for a
period of two years
 Equipment, the safety of which depends on installation or assembly
conditions, until the employer ceases to use it at the place it was
installed or assembled
 Equipment exposed to conditions causing deterioration, until the next
report is made or the expiration of two years whichever is the greatest
