Task 1

What financial arguments could you use to justify your proposed recommendations to segregate FLTs and the
workers?

Answer

Recently a worker was injured by the fork lift truck, the financial costs of the accidents are direct cost, indirect
costs, insured costs and uninsured costs. In my point of view, the financial arguments which I could use to justify
my recommendations to segregate the fork lift trucks and the workers are listed below:

Direct costs:
 The cost spent to the worker for the first aid treatment.
 The worker is off for 6 weeks, so the management has to pay the workers sick pay for the 6 weeks.
 The management has to pay the hospitalization cost for the worker.
 Compensation is paid to the injured worker as legally.
 The working time is increased for other workers as there were only 20 workers working in the
warehouse.
 While dealing with the injury the working time for the other workers is a loss for the management,
 The goods in the warehouse Is also damaged and the management has to repair or fix the damaged
goods. Some of the products could not be repaired, which causes the management has to pay the
money to the client.

Indirect costs:
 The loss of staff morale in between the management and the workers, which leads to the loss of
production and quality.
 Loss of spending in Recruiting and training a new worker in place of injured worker, as there were
only 20 workers.
 Morale for customer reduces as the goods were damaged which causes the reputation of the
business.
 The warehouse consists of hazardous chemical which leads to fire explosion.
 In warehouse, after the accident the cost spent for cleaning the oil spillage.
 The health insurance premium amount can increase due to the accident in the workplace.

By considering all these points, we can segregate FLTs and workers with barriers, pedestrian walkways,
designated crossing places and separate entrances for workers and FLTs.

Task 2 (a)

what unsafe behaviour would be included in the report?

Answers

Some of the unsafe behaviour which I have seen in the warehouse are:

 The goods are stacked in the workers walkway, obstructs the walkway, which leads the worker to
step on the forklift pathway.
 There was no record for any safe acts like near misses, incidents, preventive maintenance of Fork lift.
 FLTS operator uses fork lift while working, which distracts him and causes an accident.
 The housekeeping is poor in the warehouse, which causes the FLT to skit on oil spillage.
 There was no accident investigation team to enquire the accident.
 Safety culture not properly followed in the workplace.
 There Is no health and safety worker representative in the warehouse, to carry out their concern to
the management.
 The young drivers are unmature to handle these high risky jobs without any proper safety training.

Task 2 (b)

what voluntary feedback would be included in the report?

Answers

 Warehouse workers informed me about there is no safety practices followed in the warehouse.
 It has been a long time since the management has visited the warehouse.
 The workers complaint about the safety to the warehouse orally, there is no record of any written
complaints.
 The overall store and warehouse managers are not interested in improving health and safety in the
warehouse.
 There is no safety budget and resources available in the management, they are all concerned that I
would cause to much disruption to the business.
