based interventions at the end of their life. We strive to help patients focus less on dying and
more on living with dignity by addressing their assessed needs while providing compassionate
care.

Our Goals
To maintain the comfort and dignity of our patients while navigating end of life challenges
through meaningful, personalized and purposeful recreation.

To affirm the patients’ cycle of life by providing a platform for patients communicate and
express to their life experiences with their loved ones and present communities.

their loved ones through individualized palliative recreation and leisure options during the last
stage of their life.

7
Recreational Therapy Intervention Programs
Eating Through the Ages
The Eating Through the Ages program is designed to help process the changes of aging by
allowing patients to recreate a meal that is both personal and meaningful to them. This meal
could be something that was made for them when they were a child, a meal they would cook for
their loved ones, or even a meal from their first date with their significant other. The patient will
get to share and enjoy this meal with 7 people of their choice.
Goals
 Through this program, we will encourage patients to recall memories and follow recipes
to assist the patient in staying cognitively engaged
 To encourage residents to continue to be physically engaged with gross and fine motor
skill application, through completing the recipe with the assistance of the RT.
 To facilitate sensory stimulation to provoke emotional responses within the patients

Conversing with Companions

The Conversing with Companions program provides patients the opportunity to interact with
their loved ones and friends, as well as members of the community through conversation. This
will allow the patient to reflect upon all of their life experiences and share these experiences
which will allow their legacy to live on through generations.
Goals
 To encourage patients to reflect upon their life experiences and memories
 To empower older adults by allowing them to share wisdom with their loved ones and
people of the community
 To provide an opportunity for socialization with individuals who are not seen as medical
providers, or other hospital patients

Person-Centered Palliative Planning
The Person-Centered Palliative Planning Program is available to all hospice patients who wish to
play an active role in their time spent in hospice care. The recreational therapist will sit down
with the patient at the beginning of each week and provide them with a menu of 5-6 palliative
care treatments happening that week. There will be a variety of options which will allow patients
of all abilities to participate. Allowing patients to have this autonomy in their treatment planning
will maximize the amount of time they spend participating in activities of their choice.
Goals
 To provide patients with a variety of palliative care options each week
 To increase patient satisfaction with their life at the hospice facility by adhering to their
values and preferences
 To allow patients to exercise their right to have autonomy in developing their treatment
plan

Section: Recreational Therapy Department
Subject: Clinical Documentation

Clinical Documentation
Outlined in this section is all documentation requirements for recreational therapy patients at the
Silverado Hospice Care facility. Documentation is required for each patient in the form of
assessment, treatment planning, plan implementation, reassessment, evaluation and discharge
planning. The standards by which this department will uphold are outlined in American
Therapeutic Recreation Association’s (ATRA’s) Standards of Practice. Documentation plays an
important role of connecting the program plan and the assessment plan with measurement of
program and patient outcomes.

Assessment: A recreational therapy assessment is required for each patient which will consist of
a record review and a patient interview utilizing the Preferences of Everyday Living Inventory
(PELI). A licensed recreational therapist will conduct a reassessment every 2 weeks or monthly,
depending on the patient’s goals and objectives. The purpose of an assessment is to collect
comprehensive and accurate data, needed for the development of an individualized treatment
plan.
 Record Review:
o The recreational therapist will carry out a record review within 24 hours of
admission to the facility. This will include reviewing: the admission notes, patient
history, patient physical functioning overview, and nursing care plan.  A consent
document is provided during the intake process.
 Patient Interview
o The patient interview will be completed by the recreational therapist within 48
hours of admission to the facility. The patient interview is utilized to determine
appropriate interventions, patient strengths and limitations, patient leisure
interests, and patient’s needs while receiving hospice care. The structure of this
interview will be based off of questions located within the PELI. Patient behavior
will also be observed and documented during this interview.
 Reassessment
o A reassessment will be conducted every two weeks or monthly, depending on the
patient’s goals and objectives, through patient check-ins. Patient check-in will
consist of a follow up of the patient’s original interview to identify changes in
their desired leisure interests, strengths and limitations, and needs while receiving
hospice care.
The recreational therapist is responsible for analyzing patient assessment results and writing an
assessment summary.

Treatment Planning: The recreational therapist will utilize the assessment data to develop an
individualized treatment plan. This treatment plan will include goals and objectives to help
develop the purpose, content, and direction of the program. All patients reserve the right to be
informed of and play an active role in their treatment planning.

10
Plan Implementation: All participation in recreational therapy treatment interventions is
voluntary and patients reserve the right to refuse recreational therapy treatment interventions at
any time.  It is important to take note of the following factors:
 Interdisciplinary team plan
 Staff to patient ratio during interventions
 Attendance for documentation purposes
 Routine Documentation
o Routine Documentation: The recreational therapist is responsible for
documenting individual engagement in each intervention implementation directly
after the activity occurs using the SOAP note structure.  Documenting for each
patient will provide the recreational therapist with condensed and concise
information to help with evaluation of the intervention.

Evaluation: The recreational therapist will evaluate the patient’s progress using patient
satisfaction surveys, post session reviews with the interdisciplinary team, and feedback from the
patient’s loved ones.  Evaluation is essential to the recreational therapy process as it provides
summative data for the recreational therapist to monitor the patient’s response to interventions.

Discharge Planning:
 Within 48 hours of the medical director’s note of discharge or deceased patient, the
recreational therapist must submit a summative discharge evaluation. This evaluation
should include:
o Reason for discharge
o Interventions patient participated in
o Response to activities
 If the patient is being transferred to another facility include the following information as
well:
o Recommendations for continued care
o Level of functioning in all 5 domains
o Outcomes from interventions provided by RT

Subject: Ethical Conduct

Ethical Conduct
The Recreational Therapy Department at Silverado Hospice Care adheres to the ethical conduct
established by American Therapeutic Recreation Association’s (ATRA’s) Code of Ethics.
Abiding by the Code of Ethics will allow the recreational therapist to deliver patient
intervention/care that is professional.
Procedure:
 Once hired, each professional is required to read through ATRA’s Code of Ethics. The
performance of the therapists will be assessed annually using the ATRA structure criteria,
process criteria, and outcome criteria for ethical conduct located in the ATRA Standards
of Practice.
 Each professional is trained in the process of writing incident reports, specifically for
reporting unethical conduct to the Recreational Therapy Manager.  The recreational
therapy manager then forwards the report to the Facility Administrator who begins an
investigation.
 If unethical conduct is found by the Facility Administrator, disciplinary action will be
taken.
Code of Ethics:
 Beneficence:
o Recreational Therapy personnel shall treat persons served in an ethical manner by
actively making efforts to provide for their well-being by maximizing possible
benefits and relieving, lessening, or minimizing possible harm.

abilities, and judgment to help persons while respecting their decisions and
protecting them from harm.
 Autonomy:
o Recreational Therapy personnel have a duty to preserve and protect the right of
each individual to make his/her own choices. Each individual is to be given the
opportunity to determine his/her own course of action in accordance with a plan

with regard to their care, recreational therapy personnel have the duty to respect
the decisions of their qualified legal representative
 Justice:
o Recreational Therapy personnel are responsible for ensuring that individuals are
served fairly and that there is equity in the distribution of services. Individuals
should receive services without regard to race, color, creed, gender, sexual
orientation, age, disease/disability, social and financial status.

providing each individual receiving service with information regarding the
services, benefits, outcomes, length of treatment, expected activities, risk and
limitations, including the professional’s training and credentials. Informed
consent is obtained when information needed to make a reasoned decision is
provided by the professional to competent persons seeking services who then
decide whether or not to accept the treatment.
 Confidentiality & Privacy:

Personnel Operating Budget

The chart above indicates what the Recreational Therapy Department employees will be paid for
the 2020 fiscal year. An extensive staff is needed to provide personalized care to all hospice
patients, allowing for the development of individualized treatment plans.

Recreational Therapy Manager (MTRS, CTRS)

The Recreational Therapy Manager oversees the entire Recreational Therapy Department at
Silverado Hospice Care, including the recreational therapists. This individual has completed a
master’s degree in Therapeutic Recreation (MTRS) showing dedication and persistence in the
profession. This individual will be paid $30.00/hour to acknowledge the value of their higher
education.

Certified Therapeutic Recreation Specialist (TRS, CTRS)

Two Certified Therapeutic Recreation Specialists (CTRS) work under the Recreational Therapy
Manager. These individuals have passed the NCTRC Certification Exam after completing a
bachelor’s degree. Each CTRS will be paid $20.00/hour based on the average CTRS salary in
Utah.

Therapeutic Recreation Technician (TRT)

Each CTRS will have two Therapeutic Recreation Technicians (TRT) assisting them. These
individuals have received 90 hours of instruction from an MTRS and passed the state exam. Two
TRTs work part time to assist when four full time TRTs are not needed.

27
Supplies Operating Budget

Cleaning Supplies
The cleaning supplies has a forecasted budget of $162.00, the list above indicates appropriate
supplies required to clean the kitchen before, during, and after the program takes place. The
supplies outlined are necessary to carry out proper sanitary procedures in order to prevent the
spread of infectious disease and viral infections to ensure the safety of Silverado Hospice Care
patients.

Single-use supplies
Single-use supplies have a forecasted budget of $106.00 and are needed to assist in the cleaning
process and proper storage of food.  These supplies also aid in the preparation of many different
meals and will be needed frequently.

Food
Food has a forecasted budget of $4,800.00 which allows for a variety of meal options. Each
week an individual is given a budget of $100 to make a meal of their choice that will feed 8
people.

28
Equipment Operating Budget

Kitchen Appliances forecasted budget is:

The kitchen appliances have a forecasted budget of $150.00. The appliances outlined in the chart
above are found in most kitchens.  These are appliances that the facility does not already own
that will be needed to properly run the Eating Through the Ages program. These appliances are
used to create a variety of dishes.

29
Dinnerware and Drinkware forecasted budget is:
The dinnerware and drinkware have a forecasted budget of $108.00. Dinnerware and Drinkware
are necessary when eating and serving a meal. All individuals participating in the program will
be encouraged to sit down and eat the meal together. Enough dinnerware and drinkware need to
be purchased to serve a maximum of 8 individuals.

General Kitchen Tools forecasted budget is:

The general kitchen tools have a forecasted budget of $436.00. Many different kitchen tools are
needed for preparing a meal. A variety of kitchen tools is necessary to cook each individual's
preferred dish.

30
Patient Volume for Eating Through the Ages Program

Our total volume came out to be 1,504 patient engagement per unit.  This volume was calculated
using a 15-minute unit and forecasting for 8 patients in this program, that will occur four times a
month. The maximum number of patients that can be catered to in the Eating Through the Ages
program is 8 individuals per session.  However, it is assumed that during the holiday season
participation would be lower, so the expected number of patients per session for the month of
December was reduced to 6.

31
Activity Based Cost Operating Budget

The chart above shows the breakdown of the Activity Based Cost for the Eating Through the
Ages Program. The activity unit cost is $61.63.

Our volume budget indicated a total volume number of 1,504 participants per unit.  Using this
information and the 1:4 staff to patient ratio, it was decided that two therapists will be needed to
run the Eating Through the Ages program.  The staff cost shown above indicates that the
program will be run by one CTRS and one full time TRT employee. With all of these factors we
have identified the Activity Unit Cost to be $61.62. There are many great benefits related to this
intervention that we feel outweigh the cost associated to this event such as, social engagement,
exercise of motor skills and cognitive skills, stimulation of senses, and evoking emotional
responses. The cost of this event will be fully covered by revenue from grants, donations, and
fundraisers.

32
Capital Expenses

KitchenAid Electric Stand Mixer

The cost of the KitchenAid Electric Stand Mixer is $300.00. While this is an expensive item,
many individuals in hospice care may be experiencing atrophy and therefore may be unable to
mix ingredients by hand due to a lack of fine motor skills. Also having this item would allow
therapists to provide the best care during this program as the mixer would be able to support an
extensive list of recipes due to its many attachments.

33
Revenue
Grant(s):
Silverado Hospice Care applied for and received a federal government grant. 5% of this grant

for patients to converse with loved ones and members of the community.
 Person-Centered Palliative Planning Program
o The Person-Centered Palliative Planning Program provides patients with a menu
of 5-6 palliative treatment options for the week that aim at stimulating the senses.

Recreational therapy utilizes a holistic focus through activity-based interventions to assist in

increasing one’s quality of life for individuals in a hospice care setting.  Interventions provided
by RTs are personalized and acknowledge the individual’s input in their therapy in order to
provide the most appropriate treatment and care for the residents of Silverado Hospice Care.

The Eating Through the Ages Program

The Eating Through the Ages Program is a cooking intervention opportunity for a patient to
choose a memorable meal to cook and will be able to share this experience with 7 people of their
choice. The act of cooking and eating a meaningful meal has the potential to address the needs of
each patient. The benefits of this program include social engagement, motor skills, cognitive
skills, stimulation of senses, and emotional responses. Utilization of these skills and abilities will
work to aid in affirming the patient’s cycle of life. Through understanding each patient’s
assessed needs, the recreational therapy departmental staff will use the Eating Through the Ages
Program to help the patients focus less on dying, and more on living with dignity and comfort.

Goal of the Recreational Therapy Department Marketing and Advocacy Plan: To increase
patients’
participation in the Eating Through to Ages Program
 Objective #1: The recreational therapy departmental staff will invite, remind, and
encourage patients to attend recreational therapy departmental programs 3x throughout
the week.
 Objective #2: Once a week, the recreational therapy departmental staff will communicate
via email with other departments regarding which patients will be attending the Eating
Through the Ages Program to prevent scheduling conflicts.

64
 Objective #3: Recreational therapy departmental staff will inform loved ones of the
Eating Through the Ages Program through flyers and weekly emails.

Patient Strategy
Target Population
 The target population for this marketing strategy would be all Silverado Hospice Care
patients.
Strategy: Verbal Reminder of Activity
 On Monday mornings, the recreational therapy departmental staff will go from room to
room, inviting patients to all departmental programs. When speaking on the Eating
Through the Ages programs, the recreational therapy departmental staff should be sure to
touch on the benefits of participating in the program. These include social interaction,
exercising cognitive skills through memory recall and instruction following, stimulation
of the senses and exercising both fine and gross motor skills.
 On Monday mornings, the recreational therapy departmental staff will confirm which
patient will be choosing the meal to be prepared during the Eating Through the Ages
program for the week, and which other 7 people they would like to attend with them.
These other 7 people could include other residents, loved ones, and friends. If the patient
chose to invite other hospice care patients, the recreational therapy departmental staff will
go to their room and invite them by the end of the day. If the patient chose to invite loved
ones or friends from the community, they will be contacted via email to inform them of
the time and location of the event, as well as the meal being prepared. An RSVP will be
requested by the end of day by Tuesday night
 On Tuesday mornings, the recreational therapy departmental staff will remind the
specific patient and their chosen guests of the Eating Through the Ages Program
happening at 3:30 p.m. on Wednesday.  If the patient chose to invite other hospice care
patients, the recreational therapy departmental staff will go to these patient’s rooms and
remind them. If the patient chose a loved one or friend from the community and they
have not yet RSVP’d, a second email will be sent requesting an RSVP by the end of the
day.
 On Wednesday mornings, the recreational therapy departmental staff will get the patients
ready and excited for the program taking place later that day. The recreational therapy
departmental staff will bring aprons to the hospice care patients that are participating in
the program for the week. Other participants from the community will receive an apron
upon arrival. The recreational therapy departmental staff will inform the patient of the
week if one of their guests is not able to attend, and that all ingredients have been
purchased.

Internal Strategy
Strategy: Email Blast to Other Departments - Conversation/word of mouth
 On Monday afternoons, the recreational therapy departmental staff will send an email to
the other departments of Silverado Hospice Care, to inform them of the specific patient(s)
that will be participating in the Eating Through the Ages Program that week, in an effort
to avoid scheduling conflicts. The time and location of The Eating Through the Ages
Program will be provided in the email. The email will also ask the Silverado Hospice
Care staff to remind participating patients of this event and encourage other patients to
get involved with this program in upcoming weeks.
 The email will also include the benefits the patients will receive by participating in the
program. These benefits will help inform other staff members of the importance and the
efficacy of recreational therapy for patients. Educating on the importance of recreational
therapy in end of life care will help to advocate for the recreational therapy profession as
a whole.

External Strategy
Physical Strategy: Informational Flyer
 An informational flyer will be provided to loved ones and friends when they visit the
Silverado Hospice Care Facility. Flyers will also be updated and posted around the
facility throughout the year. A copy of the flyer can be found below.

68
Digital Strategy: Email blast to Loved ones
 An email blast informing loved ones and friends about the Eating Through the Ages
Program will be sent out once a month to encourage participation. A copy of the email
can be found below.  A copy of the flyer will be attached to the email.

The Recreational Therapy Department at Silverado Hospice Care would like to share one of our
favorite programs this year, called Eating Through the Ages, with you. Eating Through the Ages
provides your loved one with a tasty opportunity to recreate a meal that journeys them back in
time, and allow them to share it with their loved ones and friends. We love providing this
program because it brings our patients joy and happiness as they reminisce on the meaning
behind their meal.  This program is free to our patients, their loved ones, and friends! It is offered
every Wednesday at 3:30 p.m. at our Salt Lake City location; the address can be found below.
We invite you and your loved one receiving hospice care to sign up by calling or emailing us.
Please feel free to contact us with any questions you may have. We look forward to hearing from
