their respective account/group and or Dep’t.
3. No final payment shall be made by BMS unless the employee has settled all the responsibilities
and accountabilities
4. Any Losses, Damages or Shortages found upon audit will be deducted in your Final pay.
5. This clearance must be submitted personally to HRD (for Manila) to Branch Office (for
provincial area) mail it to head office (1360 FELIX HUERTAS ST. STA. CRUZ MANILA) after
completion.
6. Final pay will be released approximately 60 working days (Holidays not included) upon receipt of
this clearance by HRD.

Note: This clearance will not be processed if signature of approving authority/ party is Incomplete.
