DIGITAL CONTACT TRACING APP

What is Stay Safe?

▪ StaySafe.PH is the Philippines’ official health condition reporting, and contact tracing to
assist the government in our fight against COVID-19.

▪ National contact tracing platform was endorsed by the National Task Force and the Inter-
Agency Task Force for the Management of Emerging Infectious Diseases.

▪ Managed by the Department of Health (DOH) and is the declared Personal Information
Controller.

▪ All city and national government offices, private establishments and general public are
enjoined to use and patronize the StaySafe.ph application.
 What is Stay Safe?
All residents and visitors who will enter work or do business in all indoor or enclosed public and private

establishments in Zamboanga City shall be required to download and register at the StaySafe.ph application
and secure a unique QR code which they can store on their phone or print physical copy.

▪ Persons registered with the StaySafe.ph app shall scan the QR code displayed at the entrance using their
mobile phone; or for non-smartphone users, present the print out code for scanning with the designated
machine scanner at the entrance in all commercial establishments, workplaces, churches and government
offices in the city.

▪ In the event that the contact tracing is necessary, all government offices and private establishments shall
make available to the City Government of Zamboanga the “Digital Logbook”.

SECTION I - Zamboanga City shall adopt the STAY SAFE application as official digital contact
tracing system.

SECTION II - MANDATORY REGISTRATION FOR ALL ESTABLISHMENTS. All City and National
Governemtn offices, private establishments, and the general public are hereby enjoined to use and
patrionze the StaySafe.Ph application for digital contact tracing and utilie quick response (QR)
code on all entrances or visible spots.

For this purpose, all offices, whether government or private, private establishments and other
similar establishments shall designate a help desk or customer assistance desk for the registration
with the StaySafe.Ph or for the generation and printing of StaySafe.Ph QR Code for non-
smartphone holders.

your company/organization.
4. To add for another entry/exit points, click “Add” button and fill up required
fields and click “Create”.
