Activity 1: Physical Activity Questionnaire
The following questions will help you analyze your physical activity preference and habits. It will help you
make decisions concerning your health and fitness level. This is NOT A TEST. Answer the questions as
honestly as possible and use the scoring guide to help you assess your physical activity level.

"Excellent" if your score is 24 and above.
"Good" if your score is 15-23.
"Needs Improvement" if your score is 14 or lower.

1. I will stop checking my phone all the time - because of this pandemic, I am always on my phone,
always checking it even though there's no important things.I want to improve myself by not using my
phone and engaging in activities that can improve my lifestyle.
2. I wanted to try new hobbies like biking, planting and to play guitar - because my sister is
influencing me to try this out and to lessen the time that I spend in my phone. I also wanted to travel
but it is not possible because of the virus so I chose to stay in our house for my safety.
3. The last thing I want to improve is to sleep early - becuase sometimes I sleep at around 2 am in
the morning, it is a unhealthy habit so I wanted to improve it for myself and for my future.
