SUBJECT : Return to Work Request of Ms. Melissa Jane G. Beltran

This refers to the Memorandum dated June 21, 2021 requesting for an Alternative Work
Arrangement of Ms. Melissa Jane G. Beltran, in compliance with Department Order 83,
series of 2020 (DPWH Guidelines on Covid-19 Management).

In view thereof, the undersigned respectfully request the lifting of the Alternative Work
Arrangement of Ms. Melissa Jane G. Beltran, as he complied with the directive to undergo
RT-PCR and tested Negative, per Nasopharyngeal Swab Test and that he be allowed to
return to work starting July 12, 2021.

Attached herewith is a copy of the Negative result of COVID-19 RT-PCR Test for your
reference.
