SUBMISSION OF LDM2 FORM 1 AND FORM 2

FOR DIVISION CONSOLIDATION

Public Elementary and Secondary School Principals
All Others Concerned

1. To comply with the unnumbered Regional Memorandum dated October 29, 2020,
this office enjoins submission of the Learning Delivery Modalities (LDM2) Forms 1 & 2
outputs before 12:00 p.m. on November 25, 2020.

2. Advanced information on the google drive link of schools have already been
communicated through the division instructional coaches as early as October 29, 2020.

3. The link is a google drive where individual school can upload Form 1 per LAC leader.
Instruction on how to use the drive is attached as enclosure to this memorandum.

4. Instructional coaches may directly submit Form 2 to the SMME office for consolidation.

5. Immediate dissemination and compliance with this memorandum are desired.
