2. Narrative report:

At about 6:30 in the evening of June 24, 2021, a SELF-VEHICULAR ACCIDENT

transpired along the national highway of Brgy. Sto. Nino, Tukuran, Zamboanga Del Sur one (1) unit of
single motorcycle Mio i125, color yellow and black, bearing plate number KA 60782 with engine
number LX150CEPDO932 and chassis number MH4LX160CCKP16434, CR number 199663443 dated
11/11/2020 registered to Reynaldo Tura with postal address of Brgy. Balangasan, Pagadian city,
Zamboanga Del Sur and driven by Reymar Garcenez male, 18 years old (DOB 10/23/2003), single,
farmer and a resident of brgy. Balangasan, Pagadian city, Zamboanga Del Sur without driver’s
license.

Investigation disclosed that while traveling from west headed toward east direction at maximum
speed and upon reaching at curve lane of place of incident, subject SMC slide toward the sign post
thereby resulted to the incident. Immediately the driver was brought to Pagadian Doctors Hospital
for immediate treatment, unfortunately he was declared dead on arrival by his Attending Physician
Dr. Levi Gomez.

Furthered informed that subject SMC is now in custody at this station for proper disposition.
