Hazards or Potential Recommendations to Eliminate or Reduce
Incidents Hazards
1. Make sure the well is secured. Well could flow and blowout. Refer RMS procedure above, install barriers
necessary and B.P.V.
2. Hook up BOP winches. Hitting personnel. Ensure lifting is done slowly and include
flagman.
3. Remove rotary bushings. Slipping, dropping bushing and injuring feet. Clear below rotary and post a centry.

4. Remove bell nipple. Dropping bell nipple and fingers getting Watch for pinch points.

5. Shut down koomey and vent pressure. Pressure remaining on koomey lines. Ensure all gauges for zero reading.

6. Remove koomey pressure lines. Trapped pressure and fingers getting Be careful on lines and pinch points.

8. Using BOP winches, take slack from Falling of BOP. Use safety harness and fall arrest devices.

9. Remove bolts from BOP. Slipping, eye injury and finger injuries. Wear harness, safety glasses and watch for
pinch points.
10. Lift BOP and place on set back stump. Caught in, on or between, dropping BOP. Remain alert and clear, ensure proper lifting.
