RE: Grade 12 STEM Capstone Research

Dear Mrs. Lagahit,

Greetings!
We senior high school students currently enrolled in the STEM (Science, Technology,
Engineering, and Mathematics) Strand at Stella Maris Academy of Davao, N. Torres St., Bo.
Obrero, Davao City. We are in the process of writing our research paper in partial fulfillment of
our requirements in our Research Capstone subject. The study is entitled Factors Affecting the
Near-sightedness of Senior High School Students in Stella Maris Academy of Davao.
The simple quantitative- descriptive study aims to:
1) know the number of myopic senior high school students and if the factors listed in the
Sydney Myopia Study (SMS) are also prevalent of this school, and
2) know which of the said factors are most frequent in the school.
In line with this, we are writing to request permission to gather data in the school clinic of Stella
Maris Academy of Davao, towards the completion of the mentioned study.
We hope that the clinic will allow us to access the medical eye records of the students so that we
maybe able to calculate for our sample size. If allowed, the data gathering procedure shall adhere
to the following parameters:
a. Student identities shall be kept confidential and shall not be mentioned in our manuscript
and in our final defense.
b. The only data that we shall take is the number of myopic students in the SHS department
of Stella Maris Academy of Davao. We shall not take their names or other details in their
medical record aside from their eye health records.
Please find attached a copy of our research protocol and the prepared survey questionnaire for
your convenience. Should there be any concerns, clarifications and points for modification,

This study is something that we passionately labored for to satiate our interest and curiosity
towards health science. Your consideration and approval to allow us to gather sufficient data is
