Safety Induction before to go to check the tools. Fill of HARC. Inspection of 3 Rubicon’s NaviSHOE:

The physical structure of the naviSHOE is visually verified with legible serial number on the body
PWO-US01-A79204-04. It was observed that the integrity of the cement was damaged by
temperature effects. The baffle plate is removed from the accessory. It was observed that the
material was considerably damaged by temperature. The interior of the valve was blocked twice
when making the inspection and it tends to lean to one side and there is difficulty in its movement.
The operation of the second valve cannot be determined. The recommendation is made to change
the cement and check the operation of the valves. The corresponding pictures are attached.

The physical structure of the naviSHOE is visually verified with legible serial number on the body
PWO-US01-A79204-02. It was observed that the integrity of the cement was damaged by
temperature effects. The baffle plate was removed from the accessory. It was observed that the
material was considerably damaged by temperature. The interior of the valve is clean and there is
not difficulty in its movement. A long fracture through the entire cementum is seen on the inside
diameter of the valve path. A video was made which is attached to this email. The fracture presents
a small "wear" which is very imperceptible and cannot be seen in the photograph. The operation of
the second valve cannot be determined. The recommendation is made to change the cement and
check the operation of the valves. The corresponding pictures are attached.

PWO-US01-A79204-01. It was observed that the integrity of the cement was damaged by the
effects of temperature. The baffle plate was removed from the fixture. The baffle plate material was
found to be considerably damaged by temperature. The inside of the valve is clean and there is no
difficulty in its movement. A gap is observed in the annular between the casing and the cement in
the internal diameter. There are small cracks in the cement without being able to determine the
damage in it simply. I do not know that the cement is detached. The operation of the second valve
cannot be determined. The recommendation is made to change the cement and check the
operation of the valves. The corresponding images are attached.

PWO-US01-A79204-05. It was observed that the integrity of the cement was damaged by the
effects of temperature. The baffle plate was removed from the fixture. The baffle plate material was
found to be damaged by temperature. The inside of the valve is clean and there is no difficulty in its
movement. No gaps were observed in the annular between the casing and the cement in the
internal diameter. There are small cracks in the cement without being able to determine the
damage in it simply. I do not know that the cement is detached. The operation of the second valve
cannot be determined. The corresponding images are attached.
