Problem statement

The issue of HF is the most significant cause of incidences of hospitalization in the US. Mather

Hospital Northwell Health conducted this study so as to align the health facility with the Centers

for Medicare and Medicaid Services’ (CMS) goals reported in 2012. Thus the aim of this article

was to enhance a wide range of interventions for reducing readmissions at the health service

facility, containing the costs of services, and enhancing health for the population.

Literature analysis

Literature materials explored are resourceful and provide an informed coverage of factors

such as discharge planning, patient awareness and education, consistent communication with

healthcare service providers for outpatients, scheduling for follow-ups prior to any actions are

taken after discharge, and medical reconciliations. However, the range of resources used to

explore literature is limited and only provides for information about a factor from perspectives of

considerably few studies.

Findings

The study found that HF is one of the dominant causes of readmissions for Medicare

patients. For intervention improvement, medication review and patient education prior to

discharge help minimize HF readmissions. Also, discharge post follow-up initiatives within 3

days with heart specialists subsequently resulted in a significant reduction of readmission.

Conclusions & recommendations

The Task Force recommended the following intervention measures; proper planning,

medical education, engagement of family caregivers for education and medication processes,

organization of effective post discharge programs for following up within 3 days after

discharging, and communication.

Practical implications & applications

The most practical applications of this article are mainly improvement of mitigation

measures for cubing HF remission rates for patients. The readmission reduction insights will be

useful for managers of healthcare facilities intending to offer affordable, effective healthcare

services in order to reduce fatalities caused by HF.

Critical analysis

Despite resourceful implications of the study, the performance improvement strategies

recommended by the task force are specific to Mather Hospital Northwell Health; findings

cannot be extrapolated to other healthcare. The literature review was satisfactorily

comprehensive.
