3. Work Experience Sheet (dated within November 18-28, 2021 ), which may be downloaded from the CSC website www.csc.gov.ph;
4. Performance Rating I Performance Evaluation (IPCR/DPCR) in the last rating period (if applicable};
5. Ori~inal Authenticated Certificate of Eligibility I Certification of Bar or Board Examination Results, Rating and License Card;
6. Appropriate License as to requirement for the position (i.e., Professional Driver's License, CSC MC No. 11, s. 1996, as amended by CSC MC No. 13, s. 2013- Category I, II, Ill and IV);
