TASK 1: JUSTIFYING HEALTH AND SAFETY IMPROVEMENTS
QUESTION 1

The financial reasons relate to fact that accident and ill-health cost money. When accident occurs there
will be a direct and indirect cost and some of these costs are insured against and another is uninsured
against. Insurance is not a solution to cover all loss.

ILO statistics show that every 15 seconds worker dies, 153 work-related accident. That mean 6,300
people die daily and yearly it’s more than 2.3 million.

The segregation of FLTs and workers lead to:


– Prevent financial direct loss. The consequence of collision between FLT and passing worker are: first
aid, hospital charges, worker sick pay, compensation payable and the collision between FLT and
products cause damage and loss of materials. All that will cost money and decreasing the profit.
– Prevent financial indirect loss as a previous example by: loss of time for investigation, recruitment and
training.
– Prevent enforcement action that lead to prosecution in criminal court, fine or/and imprisonment as a
result of administration’s indifference to OHS, which causes an increase in the percentage of accidents
and compensation claimants, which leads to the orgnisation’s bankruptcy.
– Prevent loss of employee’s confidence. Based on the scenario, the absence and resignation of workers
is on the rise due to their insecurity in the warehouse, and this may cause a decrease in productivity,
which leads to a decrease in profitability.
– Increase in Profit as a result of segregation of FLTs and workers which lead to reduce accidents then
the profit will be increased.
– Increase in reputation by avoiding enforcement actions. That will not be if the manager provides safe
work place and safe system of work to prevent worker absence and turnover.
– Workers confidence & happiness when the segregation are done because they safer when
carrying out their work and make them feel the management cares for them.

TASK 2: CHECKING MANAGEMENT SYSTEM EFFECTIVENESS

TASK 5: INFLUENCING HEALTH AND SAFETY CULTURE


QUESTION 5

The negative indicators of safety culture at the warehouse are:

•Accident records: Accident records can be used as an indicator of a negative health and safety
culture as the incidence of accidents increases. As stated in the scenario, the number of
accidents and injuries was high during the past years, and some accidents are not reported.

•Absenteeism: The high rate of absenteeism is evidence of a negative health and safety culture
because it indicates that workers are unable to continue working due to suffering and ill health
at work. Based on scenario, workers absence is high in the warehouse.

•Staff turnover: The workers feel unsafe due to the lack of training, lack of counseling by the
manager and unsafe workplace. Therefore, the workers do not stay long in the organisation as
stated in the scenario.

•Compliance with safety rules: Workers do not act safely and do not comply with safety rules
and there is no audit or inspection. Workers often break rules due to poor supervision. For
example, the goods are not on the shelves, the use of forklifts in an unsafe manner, the oil
spilled on the floor of the warehouse.

• Workers' complaints: complaints are not activated effectively. Therefore, many complaints
were submitted and were not taken seriously, in addition to the absence of complaints records
or a representative of the workers.

TASK 6: DEVELOPING SAFE SYSTEMS OF WORK (SSoW)


QUESTION 6

•We need to know nature of work for the construction site and the hazards that might be occur
in the workplace. For example, physical strain, moving machine, loading and unloading, falling
objects, hazardous substances, lack of safety training, Slips, Trips, and Falls.
•The general risk level of the workplace must be considered. This is high in warehouse a result
of negligence and non-compliance with occupational health and safety.
•We should ensure that the legal requirements are met to prevent accidents by providing first-
aid and first-aiders.
•The number of workers and accident history is important because they determine the number
of first aider and first aid facilities that the site need. For example, the number of employees is
22 (20 workers or supervisors).
•Vulnerable persons, work patterns and shift systems must be known to distribute the first
aider shifts due to work needs and worker safety. For example, a young worker works on a
forklift and the work pattern is two shifts, and each shift is 12 hours.
•The first Aid equipment should be provided and inspected to ensure about availability of
equipment in first aid boxes.
•The Communication methods must be provided to contact relevant authorities as it needed.
•The workplace location is important if it easy to reach or not and that determined the
capabilities of first aid needed. For example, the hospital is 5 miles away from store. So, the
workplace needs to intensify efforts to raise the efficiency of first aid and to ensure the
availability of paramedics
•The workplace should be provided by appropriate first aid rooms and facilities with easy access
and environmental conditions such as heating, lighting, ventilation, treatment, lying, clinical
waste disposal, Hand-wash facilities.
•Review process is needed to ensure about work effectiveness and availability of first-aid
facilities.

TASK 7: INVESTIGATING INCIDENTS
QUESTION 7

First, the information required to write the accident report must be recorded: the name of the worker,
the date and time of the accident and a brief description of the accident.

Because the FLT driver did not reach the legal age and the accident caused by him being busy with his
mobile phone after the FLT slipped due to oil spilling on the floor, which led to the goods falling on the
worker. So, a high level of investigation must be carried out.

Then, all the points mentioned clearly indicate a high probability of accident. This means that the
probability is high (certain).

The previous points indicate that the risk is high, which may lead to severe wounds and fractures, or
cause the death of the victim.

This means that the severity is high (significant or fatal). Therefore, all data indicate a high-level
investigation (red).

Therefore it is necessary to form a task force under the supervision of top management or managers to
investigate.

The team consists of supervisors or executives, health and safety advisors, and worker representatives
in order to find out the causes and determine the actions needed to resolve them as quickly as possible.
