SUBJECT : Conduct of Employees’ Annual Medical Examination

The following personnel included in the attached list are hereby ordered to report to the
Employees’ Health and Wellness Center (located at the 6 th floor, Wellness Building,
JBLMGH) to undergo Annual Medical Examination for the year 2022 on their designated
schedule. Compliance to this order is a requirement by the Civil Service Commission for
government employment purposes. Compliance to the designated schedule MUST be strictly
observed. If for some valid reason, an employee will be unable to undergo medical
examinations kindly submit an explanation letter address to the Chief Administrative Officer.
Re-scheduling for valid reasons will only be allowed once.

Guidelines for Annual Medical Examinations:

Employee will be notified by his/her respective Department/Section/Unit head that

he/she is scheduled to undergo annual medical examinations.

For Day 1:

1. Employee must observe all requirements for the diagnostics procedures such
as fasting (for those employees 35 yrs. old and above, otherwise fasting will not be
required).

2. On the designated day that the employee is scheduled, the employee must proceed to
the Laboratory Department, Radiology Department, Dental Clinic, Heart Station, and
Obstetrics and Gynecological Department (if applicable) bringing the Annual Medical
Exam Checklist form. Employee shall write his/her details on the logbook provided
by the respective diagnostics services.

Required Diagnostics are restricted only to the following procedures:

a) For employees below 35 years old:

i. Complete Blood Count with Platelet Count
ii. Routine Urinalysis
iii. Chest Radiography (PA View)

b) For employees 35 years old and above:

i. Complete Blood Count with Platelet Count

c) For women employees of reproductive age

i. Pap smear

3. Laboratory, Radiology, and Heart Station services will release the results of the
examinations on the same day they were done to the Occupational Health Nurse on-
duty at the Employees’ Health and Wellness Center.

For Day 2 (next working day of the scheduled date of AME):

1. Employee must report to the Employees’ Health and Wellness Center for updating of
medical records (all employee medical records will be kept in the Employee Health and
Wellness Center for safekeeping) and to accomplish the Annual Medical Examination
Form and interpretation of test results. In observance of the Gender and Development
program, female employees may opt to be assessed by a female occupational
physician if necessary.

2. Employees with co-morbidity and/or findings that warrants additional work-ups are
advised to undergo additional examinations.

3. Failure to undergo examinations on the designated schedule shall be notified to

report to the office of the Chief Administrative Officer.

SCHEDULE OF OPERATING HOURS OF DIAGNOSTIC SERVICES AND DEPARTMENT

INVOLVED IN THE PROCESS OF ANNUAL MEDICAL EXAMINATION:

1. LABORATORY DEPARTMENT for blood extraction

6:00AM to 9:00AM – for employees 35 years old and above
1:00PM to 3:00PM – for employees below 35 years old

2. RADIOLOGY DEPARTMENT for chest x-ray

7:30AM to 12:00NN – all employees

3. DENTAL UNIT for dental check-up

8:30AM to 4:00PM – all employees

updating of medical records, submission of the accomplished checklist and releasing
of Annual Medical Examination Certificate (JBL-EHW-FM-08C)
