This Exit Report, as required under DOH Department Order No. 2022-0451, provides all Human Resources for
Health under the National Health Workforce (HRH-NHWSS) with an opportunity to give feedback which may assist
the Department of Health (DOH) in the improvement of the Human Resource for Heath Deployment Program. The
exit report may be completed in association with an exit interview. All reports are reviewed to determine if follow up
action is required.

Please complete the questionnaire in all honesty, as your answers may affect decisions made in relation to the HRH
Deployment Program.
