SY 2021-2022
I. INTRODUCTION
The following obligations and functions as a Grade 10 Jacinto adviser, English subject
teacher, and English club, Reading Coordinator, and School ICT coordinator for the month of
October.
The following are the highlights of accomplishments for November in each key
result area of development.
A. Teaching-Learning Process
1. Prepared and executed Weekly Home Learning Plan in English Grade 7 and G10.
2. Prepared and used Instructional Materials (ICT Based) in English Grade 7 and
Grade 10 subjects for those students with internet connection.
3. Conducted weekly online classes to those students with a strong internet
connection (Grade 7&10 English).
4. Prepared supplementary materials to those students who do not have internet,
gadgets, and the like.
5. Facilitated the distribution and retrieval of SLMs/LASs and Answer Sheets as
well as the summative tests.
A. Students’ Development
1. Encouraged the students to watch or listen to the episodes of the Radyo Eskwela
sa Isabela every Wednesday.
4. Conducted orientation on how the use of LASs, SLMs, google meet, google form,
and the like.
5. Conducted the month-long Reading Celebration 2021 which lighted the Araw ng
Pagbasa. Completion Report of the conduct was accomplished.
1. Assisted the advisers in the updating and checking of students’ profiles in the LIS.
3. Conducted meeting to the English Club officers for the celebration of Speech Fest.
D. Community Involvement
2. Led the greening program of the barangay being assigned as the Purok Leader.
III. Conclusion
Indeed, through dedication and hard work every task shall be accomplished
successfully. The aforementioned accomplishments were made possible through the
leadership of the school head.

I. INTRODUCTION
The following obligations and functions as a Grade 10 Jacinto adviser, English subject
teacher, and English club, Reading Coordinator, and School ICT coordinator for the month of
October.
The following are the highlights of accomplishments for December in each key
result area of development based on the assigned designations:
A. Teaching-Learning Process
1. Prepared and executed Weekly Home Learning Plan in English Grade 7 and G10.
2. Prepared and used Instructional Materials (ICT Based) in English Grade 7 and
Grade 10 subjects for those students with internet connection.
3. Conducted weekly online classes to those students with a strong internet
connection (Grade 7&10 English). There are 55 learners attending the scheduled
online class.
4. Prepared supplementary materials to those students who do not have internet,
gadgets, and the like.
5. Facilitated the distribution and retrieval of SLMs/LASs and Answer Sheets.
A. Students’ Development
1. Encouraged the students to watch or listen to the episodes of the Radyo Eskwela
sa Isabela every Wednesday.
4. Conducted orientation on how the use of LASs, SLMs, google meet, google form,
and the like.
5. Encouraged the learners to join the contested activities in the 20th Founding
Anniversary of the school.
3. Conducted meeting to the English Club officers for the celebration of Speech Fest.
D. Community Involvement
2. Maintained the barangay greening program’s area as part of the task as the
assigned Barangay Purok Leader.
III. Conclusion
Indeed, through dedication and hard work every task shall be accomplished
successfully. The aforementioned accomplishments were made possible through the
leadership of the school head.

I. INTRODUCTION
Despite the Academic Health break for the month of January, still I was able to
perform the tasks assigned for the remaining days of reporting to school for this
month.
The following are the highlights of accomplishments for January in each key
result area of development.
A. Teaching-Learning Process
1. Prepared and executed Weekly Home Learning Plan in English Grade 7 and G10.
2. Prepared and used Instructional Materials (ICT Based) in English Grade 7 and
Grade 10 subjects for those students with internet connection.
3. Monitored the students in the conduct of the Summative Test for the second
quarter
B. Students’ Development
3. Conducted quizzes and seatworks ( in print and digital-google form) to assess the
students’ learning in this new normal.
C. Curriculum Development/ Staff Development

result area of development.
A. Teaching-Learning Process
1. Prepared and executed Weekly Home Learning Plan in English G10.
4. Conducted online class to the Grade students who have internet access.
B. Students’ Development
3. Provided additional tasks such as essay writing, speech delivery, and reading.
4. Checked students outputs and provide feedbacks thru their group chat for
improvement.
9. Prepared MOVs during the Monitoring done by the SDO Team on the
implementation of the School Reading Program
D. Community Involvement
1. Worked with the people at purok 1 for the beatification in the designated area.
