In the following table, you will find a list of possible interview questions. At the top of the table is a response rating scale. Please use this scale throughout the
interview to assess the student’s competency and depth of their responses. If you choose to forgo the provided questions, please indicate your questions
(copy/paste new or delete any irrelevant questions). Please use the 5-point competency scale as well as qualitative comments to report the student’s
