AFFIDAVIT OF SELF-ADJUDICATION
I, LUIS MANALONZO, single, of legal age, Filipino citizen, with residence and postal
address at #29 Military Cut-Off, Baguio City, after having been duly sworn to in accordance with
law, hereby depose and state:
1. That my sister, LUISA MANALONZO, died intestate in Cook, Chicago, Illinois on the 5 th
day of March, 2007 leaving the following described property in Transfer Certificate of
Title No. T-65743:
A Parcel of land (Lot 96-A-107 of the subdivision plan, Psd-15532, being a portion of Lot
No. 95-A described on plan Psd-14567, G.L.R.O. Record No. 12066), situated in the Residence
Section “K”, City of Baguio, Bounded on the NE, by Lot No. 95-B-9 of the subdivision plan; on
the SE, by public land; on the SW, by Lot No. 95-A-plan. Beginning a point marked “1” on plan,
being N.31 deg 34W, 116-18m.
Xxxx thence S. 30 deg. 26’E, 23.22m. to point 2;
thence S. 30 deg. 15’W, 20.22m. to point 3;
thence N. 30 deg. 08’W, 21.76m. to point 4;
thence N. 30 deg. 10’E, 29.22m. to point of beginning;;
2. That the decedent left no known debts; neither has she left any ascendants or
descendants or any other heirs, except me.
3. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate unto
myself the above-described real estate by means of this Affidavit and hereby files the same
with the Register of Deed of Baguio City with the request that the said adjudication be made
effective without judicial proceeding as prescribed by the aforementioned Rules of Court.
