DIALOG ABOUT URINARY CATHETERIZATION

The nurse on duty at night performs the service to the nurse in the morning.
The night nurse gave instructions to the morning nurse to insert a catheter into Mrs.
A
The nurse on duty tonight is a bed 3 patient. A entered at 06.00 WIB with
complaints of pain in the bladder, and unable to urinate. The doctor on duty
intervened to conduct an assessment of Mrs. A and insert a catheter

PRE INTERACTION
The nurse sees the patient's status, then the nurse wants to assess the patient.

INTERACTION
The nurse goes to the patient’s room
Nurses 1 and 2: Good morning mother.
Patient: Good morning Nurse!!
Nurse I: Introducing ma'am, my name is Netasya Anggraini, I'm on duty today from
7 o’clock until 2 o’clock.
Nurse 2: Introducing ma'am, my name is Miftah Irfina who is on duty from 7 to 2
later. This morning, my colleague and I will conduct an assessment of the mother
before we insert the catheter into the mother. The purpose of this study was to
determine the state of the mother's bladder. It's about 15-20 minutes. It's just here.
Are you ready?
Patient: Yes nurse, I am ready.
Nurse 1: is the mother in this lying position comfortable? (looks at with concern)

Nurse: I'll start, ma'am. I will measure the mother's pain level. I have numbers from
0-to 10. Mom, show me the number of times you feel pain
Patient: in number 6 Nurse
Nurse: it means that the pain is moderate, what are you feeling now?
Patient: my stomach hurts, I barely slept all night, then my bladder feels full.
Nurse: ok ma'am, I will feel the part of your bladder, sorry ma'am.
Nurse: Mother, I will close the skerem first, ma'am, so that you feel comfortable.
Patient: Fine sus (Nurse acts).
Nurse: I started doing it, ma'am. Please excuse me. I want to palpate your bladder.
If you feel sick, take a deep breath and just tell me. (nurse teaches deep breathing
technique)
Patient: oops sick
Nurse: take a deep breath, ma'am, as I taught
Patient (takes deep breaths)
Nurse: how are you, the pain has lessened?
Patient: yes, it's better Nurse, thank you
Nurse: you're welcome, ma'am, have you finished studying your bladder. Later my
colleague will install a catheter, ma'am, so that you can urinate and your stomach
doesn't feel full. ok my mom excuse me first
Patient: yes nurse.

1. Pre Interaction
The nurse looks at the patient's status before interacting with the patient. The nurse
studies the patient's status. The nurse explores feelings of fantasy and self-
power/self-evaluation. The nurse re-examines the examination equipment. The
nurse writes a conversation plan or activity that will be carried out during the
interaction. if the nurse is ready to plan interactions with clients.

2. Orientation Stage
Nurse 1&2 Excuse me, good morning ma'am.
Patient: Good morning nurse.
Nurse 1: Is it true that the mother's name is Sarah
Patient: That's right, I'm Sarah.
Nurse 2: Introducing my mother and my colleague nun. According to the contract
with my colleague earlier, I will start installing the catheter, ma'am. Are there any
questions before we take action?
Patient: Approximately how long will it take for the catheter insertion?
Nurse 1: Insertion of this catheter takes about 15 minutes Mrs.

Nurse 2: Then wait a moment, I'll take the tool first.

3. Stage of Work (the nurse returns to the room with a catheter insertion tool)
Nurse 2: we start now, ma'am. (while closing the creme) Patient: But nurse, this is
the first time I've had a catheter inserted so I'm still scared.
Nurse 1: Sarah, you don't have to be afraid. It hurts a little, but you don't need to
worry because deep breathing exercises will reduce the pain in your vital organs
when the device is inserted. As I have taught earlier.
Patient: ok Nurse
Nurse 2: OK, now I'm starting to help insert the catheter. Can you open it a little to
the side of the foot? To make it easier to insert the tool into the mother's genitals.
Patient: Fine, you can. (carry out the catheter insertion procedure according to the
format
Nurse 2: Hold on a bit, ma'am, take a deep breath. I'm trying to slowly insert the
device into the mother's vital organ.
Patient: (deep breathing exercises many times.
Nurse 2: Okay, it's finished (while tidying up the tools) How did you feel after the
catheter was inserted? (nurse while tidying patient "reassemble"
Nurse 2: Okay, finished (while tidying up the equipment) How did you feel after
the catheter was inserted? (the nurse while tidying the patient puts back the blanket
so that the patient is comfortable and maintains his privacy)
Patient: Suffering pain and discomfort, but my stomach feels relieved after this
catheter is inserted.
Nurse 1: Yes, the mother will feel uncomfortable at first, but in the next few days
she will get used to the presence of this catheter in the mother's bladder. Look, the
catheter bag already contains Sarah's mother's urine. Sarah's mother's urine came
out 500 cc. Because Sarah's mother has now had a catheter installed, if she ever
wants to urinate, there will be no more pain and her urine will come out on its own
through the tube to the catheter bag. (the nurse shows how many ce of urine Sarah
came out).
Patient: So that's how the catheter bag works, thank you, nurse.
Nurse 1&2: Yes, ma'am, if there is something you need, please press the bell or
your family can call the nurse at the nurse station. Have a good rest, ma'am.
(nurse smiles and leaves the patient)
