WHAT HAPPENED ? PHOTO/SKETCH:

 On the day of incident, chimney (foundation)


de-shuttering activity was planned.

 The height of the chimney (foundation) was


approx. 1 m from the sloped ground surface.

 The worker used wooden runner piece to


climb on the chimney (foundation).

 While climbing, he got slipped and lost his


balance.

 Fortunately he hold the reinforcement and


prevent himself from falling on the ground
surface.

WHAT WENT WRONG?


 Wooden runner piece was used instead of ladder to climb on the chimney
(foundation). (SOP violation)

 Effective supervision of work was not ensured.
