Incident reporting and investigation Procedure

Incident investigation shall be carried out by pre-determined investigation team. HSE MR or

his delegate shall be lead the team and guide them throughout the process. Other team
members can include relevant Division/ Department/ Project Manager or their delegates,
relevant supervisors, and workers, or, where they exist, workers’ representatives. HSE MR
shall ensure that at least on member of the investigation team is competent in Incident
Investigations. He shall be able to advise the team to carry out the processes involved in
incident investigation, in particular, he shall be able to use root cause analysis techniques.

iii. Incident Investigation process

TNC shall follow a four step approach when investigating HSE incidents. These steps are:

a. Gathering Information

Incident investigation shall start as soon as possible after the incident has happened
as the information will more reliable if acquired in timely manner.

Information can be gathered by inspecting the accident site, interviewing the

witnesses, and reviewing the documents and records related to the work location,
activities being performed there, and persons present at the location before and at
the time of the incident.

CCTV footage, entry log book, employees’ medical records, plant and equipment
maintenance records, worksite or plant and equipment inspection reports, reported
hazard observations and previous incidents, plant and equipment user manuals,
MSDS for materials, Permit-to-work, if applicable, Risk Assessments, etc. can be

clients.

Hazard observations shall be reported by filling and submitting HSE-10-1-1-5 Hazard

Observation Form, digitally or physically, to the nominated persons.

Respective Division/ Department/ Project Manager shall assign persons to review the
submitted hazards observations, and take actions to correct the situation. Summary of
hazards observations reported through these forms and any other mean shall be provided
to HSE MR for analysis and evaluation.
