Webinar Speaker Confirmation and Agreement

This speaker agreement is designed to set clear expectations for the delivery of all content
related to the webinar.

Webinar Information

1. Webinar will be 45 minutes in duration, including time for a question and answer
session at the end.
2. Webinars will be on Thursdays from 11:00 am – 11:45 am (GMT+1).
3. Webinar viewers are strongly encouraged to join the live webinar broadcast. There
will be an option for those who register for the webinar to view the recording at a
later time. Actual participation will vary.
4. Members of Five4 Media team will be online for support with the webinar delivery
platform.

As a Webinar Speaker, you will be required to:

1. Speak according to the scheduled date and time. If you must cancel, you are
required to notify Brian Bates (bbates@ltcillinois.org) no less than one week from
the scheduled session.
2. Have access to a computer with a strong Internet connection to conduct the
webinar and additional equipment such as a webcam (preferred, but not required),
speakers, and microphone.
3. Please submit marketing materials (headshot and a brief description of your
presentation) with the signed agreement within one week of receiving this
agreement.
4. Provide an agenda and any presentation materials, including PowerPoint (PPT),
Google Slides, PDFs, or any additional supplemental handouts, videos, etc. no later
than one week prior to the date of the webinar.
5. Assist in the promotion of your webinar, to the best of your ability.

As a Webinar Speaker, you agree to the following terms:

1. All webinars will be recorded for archive purposes and made available in the
Webinar Library of the LTC website.
2. You attest that you are the author/owner and/or you are authorized to use the
content to be presented and that LTC is not liable for any content presented or
misused on your part.
3. You grant permission to LTC to publish your title and description for public viewing
on the internet and in appropriate promotional materials.
4. You give permission to LTC to provide the presentation handout materials
developed for the webinar to registered attendees.
5. Webinar speakers are still the owners of the intellectual property.
6. You have obtained written photo release permission for any photos of individuals to

be used for the purpose of the presentation and handout for the LTC webinar and
for the purpose of marketing the webinar (e.g. email, web posting, social media
posting or printed materials.) Graphics featuring or including children, other than
purchased stock artwork, must have written permission from a parent or guardian
for LTC to use the graphic.
7. The LTC Webinar Series includes both the live broadcast and any archived version
of the broadcast.
