PURPOSE
The main objectives of the Customer Acceptance Procedure (CAP) are to:
• Provide Varian the opportunity to demonstrate that the Clinac was designed,
delivered, and operates according to specifications listed in this document.1
• Familiarize the customer with the performance, daily checkout, and general
maintenance of the Clinac.
SCOPE
The Clinac System Verification documents are developed and approved by Product Support
Engineering (PSE) based on inputs from the Engineering System test procedure and
summary, RAD specifications documents, and Customer Support Installation procedures.

GENERAL PROCEDURE
The CAP consists of a series of specifications and testing procedures. The tests can be
performed either in sequential order, all at once, or in an alternating sequence during logical
periods of the machine installation. The CAP is a subset of the System Verification
Procedure and Summary.
The System Verification Procedure and Summary (SVP & SVS) are accomplished and
certified by the Varian Customer Support Installation Representative during the installation
of the product. Verified data from the SVS may be directly transferred into the CAP at the
customer’s discretion.
The Dosimetry portion of the System Verification Summary should be accomplished during
the CAP. See the Note in the Dosimetry section regarding available worksheet options.
All positions are listed in IEC scale with Varian (VAR) scale in parentheses. When IEC-601
differs from IEC-1217, then they will be called out separately.
All data entry boxes must be filled in. If any boxes do not apply, enter NA.
Many measurements simply require a checkmark to indicate the test has met the required
specification. It is not required to measure and record a numerical value for these tests.

Inplane: the vertical plane defined by the accelerator gun and target; equivalent to Radial.
Crossplane: the vertical plane that is at right angles to the Inplane surface; equivalent to

Varian can provide a Wellhofer Buddleship scanning system for USA customers. Customers outside of the USA are
required to provide a scanning system for acceptance testing. However all specifications and procedures are written
for the Wellhofer Scanning System using the Varian protocol. If another scanning system is used and a conflict in the
measurements occurs, the tests should be verified using a Wellhofer Scanning System.

1. RADIATION SURVEY
As soon as the Clinac is able to produce a radiation beam, a qualified radiation expert
provided by the customer shall conduct a survey. In addition to direct beam testing, a
scattering material such as water or solid phantom must be placed in the radiation beam to
perform secondary barrier testing. To obtain worst-case scenario, the following parameters
will be used as minimum conditions:
• The Clinac must be operating at the maximum dose rate
• The Gantry must be tested at selected angular positions with the beam on
• The Collimators on the treatment head must be set for the maximum field size
If dose rates in areas external to the treatment room exceed radiation levels recommended
by the governing agency, machine beam operation is to be immediately suspended until
either the equipment, or the facility is modified or a temporary deviation issued. A
temporary deviation may consist of operating at a limited dose rate, operating with the
Gantry restricted to certain positions, or a combination of both as determined by the
qualified radiation expert. This survey is a preliminary check used to determine temporary
safe installation environment. It is not to be used as the data gathering survey that will be
performed by the customer after installation.

1.1. Collimator Transmission

Specification
• Transmission through the moveable Collimators shall not exceed 0.5% of the
central beam intensity. Collimator Transmission tests are performed during
manufacturing testing of the system. Results of this testing are available
from Varian upon request.

1.2. X-ray Leakage

Specification
• The average X-ray intensity measured over an area of 100 cm2 at a distance
of 1 meter from the primary beam shall not exceed 0.1% of the intensity at
isocenter. Data is on file at Varian that represents a typical X-ray intensity for
the High Energy Clinac system. This data is available from Varian upon
request.

2. MECHANICAL TESTS

2.1. Mechanical Isocenter Variation with Rotation

It is not possible to calculate precise values using the following isocenter


test methods and equipment. It is only necessary to verify that the rotational
isocenter meets specification. Therefore, after performing the following
isocenter tests, enter a “< value” instead of an absolute value in the
Mechanical Isocenter Measurements table (for example if the measurement
appears to be between 0.5 and 1.0mm radius, enter <1.0mm)
If precise measurements are desired, the Customer can perform these tests
at their leisure after acceptance.

2.1.1 Collimator Rotation

Specification

• The mechanical rotational isocenter for the Collimator axis shall be


confined to a sphere of ≤1.0 mm radius.

Test Setup
1. Level the Gantry at 0° (180° VAR). Rotate level end-over-end to
check level accuracy.
2. Install a calibrated 100 cm front pointer.
3. Extend the tip of another front pointer over the front edge of the
Couch and rotate the Gantry between 90° to 270° to accurately set
the distance of the Collimator front pointer to precisely 100 cm TSD.
4. Remove the Couch top front pointer.
5. Tape a piece of 1mm ruled graph paper to the top of the Couch.
6. Position Couch top until the graph paper is just below the front pointer
(but not touching).
7. With the graph paper aligned under the front pointer tip, rotate the
Collimator from 90° to 270°, while observing the pointer run-out.
8. If the walkout is >0.25 mm, shim or adjust the front pointer as
necessary to minimize run-out.

Although the Collimator rotational isocenter spec is ≤1 mm


radius, the front pointer must be aligned for ≤0.25 mm run-out
on the front pointer to properly test the mechanical isocenter
tests that follow for the Couch and Gantry. After adjustment,
do not disturb the front pointer.

Method of Test
1. With the graph paper aligned under the front pointer tip, rotate the
Collimator from 90° to 270°, while observing the pointer run-out.
Verify that the worst case run-out meets specification.
2. Fill out the applicable box in the Mechanical Isocenter Measurements
table.

2.1.2 Gantry Rotation

Specification

• The mechanical rotational isocenter for the Gantry axis shall be


confined to a sphere of ≤1.0 mm radius on Clinacs without a
Beamstopper or ≤2.0 mm radius with a Beamstopper.

Test Setup
1. The front pointer, Gantry and Couch should be set-up per the
previous step with the collimator at 0° (180° VAR).
2. Tape the short front pointer (or a 2-mm drill bit) to the end of the
Couch so that the 2-mm tip is extended over the end of the Couch
top.
3. With the Couch at 0° (180° VAR), position the Couch vertical axis so
that the tip of the Collimator front pointer is aligned to the center of the
short front pointer tip.
4. Position the Couch longitudinal axis so that the tip of the short front
pointer is approximately 1 mm away from the Collimator front pointer.
5. Position the Couch lateral axis so the tip of the short front pointer is
centered on (but not touching) the tip of the Collimator front pointer.

Method of Test
1. Rotate the Gantry through a full 360° while visually checking the front
pointer run-out every 90°.
2. Verify the front pointer tip run-out is ≤1.0 mm radius throughout the
entire 360° of rotation for counterweight systems (or ≤2.0 mm for
Beamstopper systems)
3. For systems with a retractable Beamstopper, perform this test with the
Beamstopper retracted, and again with it extended.
4. Fill out the applicable boxes in the Mechanical Isocenter
Measurements table. Enter NA in any boxes that do not apply.

2.1.3 Couch Rotation

Specification

• The mechanical rotational isocenter for the Couch axis shall be


confined to a sphere of ≤1.0 mm radius on Clinacs without a
Beamstopper or ≤2.0 mm radius with a Beamstopper.

Test Setup
1. The front pointer, Gantry and Couch should be set-up per the
previous section with the collimator at 0° (180° VAR).
2. Rotate the Couch to 90° and align the graph paper under the front
pointer.

Method of Test
1. Slowly rotate the Couch from 90° to 270° while observing the front
pointer run-out every 45°.
2. Verify the worst case run-out is ≤1.0 mm radius for counterweight
systems and ≤2.0 mm for Beamstopper systems.
3. Repeat test with Beamstopper extended for systems with a
Beamstopper.
4. Fill out the applicable boxes in the Mechanical Isocenter
Measurements table. Enter NA in any boxes that do not apply.

2.2. Field Light & Crosshair Alignment

2.2.1 Field Light Alignment

Specification

• The edges of the light field at 100 cm TSD shall be parallel and
orthogonal within ±1° (1° equals 5.2 mm of linear error on a 30 cm
field).

• The Field Light source shall intersect mechanical isocenter within ≤1.0
mm at 100 cm TSD.

Test Setup
1. Verify Gantry is at 0° (180° VAR).
2. Open the jaws to 35×35 cm.

Method of Test

Since the jaws are not mechanically linked, the jaw edges
cannot be used to verify Field Light source alignment.
Instead, an independent shadow source must be used that
will not rotate with the Collimator.

1. Using an external shadow source, verify ≤1.0 mm run-out in the light


field projection. This test can be accomplished as follows:
1) Fully raise the Couch with the readouts set to raw scale.
2) Position a sharp-edged ruler over the end of the Couch top until
the ruler is near the center of the field. The projected shadow
should be on the turntable.
3) Place a sheet of graph paper on the turntable and align the graph
paper with 2 perpendicular edges of the ruler shadow.
4) Reset scale to IEC or Varian and rotate the Collimator from 90° to
270°. Verify the shadow edges do not run-out by more than
1.5 mm (1.5 mm on the turntable equals 0.65 mm at isocenter).
5) Fill out the applicable box in the Field Light & Crosshair Alignment
table.

2.2.2 Crosshair Alignment

Specification

• The crosshair intersection shall intersect mechanical isocenter within


≤1.0 mm at 100 cm TSD and the crosshairs shall be parallel with the
Collimator jaws within ±2.5 mm as measured at the edges of a 35 cm
field. See following Note.

Although it is only necessary to meet the specifications listed


above for the crosshairs, it is actually necessary to align the
crosshairs much more accurately for Clinacs with an MLC
and/or OBI.
To avoid complications later when calibrating the MLC or the
imaging system, the crosshair intersection run-out should be
≤0.5 mm and the crosshair line used for MLC leaf calibration
should be parallel to ≤0.5 mm.
However, crosshair lines may not always be exactly 90°
perpendicular. Therefore, accurately align the parallelism of
the Varian MLC crosshair reference line and accept the
larger error, if necessary, on the other crosshair line.
Brainlab Micro-MLC’s are referenced to the other crosshair
line. But since only a 10 cm central section of the line is
used for leaf calibration, a parallelism error of 2 mm at 35cm
will result in <0.3 mm of leaf bank skew which is well within
clinical requirements.

Test Setup
1. Verify Gantry is at 0° (180° VAR) and open the jaws to 35×35 cm.

Method of Test
1. Set the Couch top to 100 cm TSD and tape a piece of graph paper
ruled in millimeters to the Couch top.
2. With the crosshairs aligned on the graph paper, rotate the Collimator
from 90° to 270° and verify crosshair run-out is ≤1.0 mm at isocenter.
3. Verify each crosshair is parallel to the upper and lower jaws. Perform
this test as follows:

1) Position the Collimator to the center position and align the


crosshairs to the graph paper.
2) Set the upper jaws to 35cm and independently drive each of the
lower jaws until both jaws are 1 cm away from one end of the
projected crosshair line.
3) Measure the distance from the crosshair to each jaw at the other
end of the crosshair line and verify the worst-case error for the
radial crosshair line is within specification. This line should be as
accurate as possible for MLC leaf calibration.
4) Repeat this test for the upper jaws with the lower jaws set to
35cm. Verify the worst-case error for the transverse crosshair line
is within specification.
5) Fill out the applicable boxes in the Field Light & Crosshair
Alignment table.

If an MLC is also being installed, enter this data in the MLC


CAP at this time.

2.3. Jaw Position Readouts

2.3.1 Asymmetric Mode

Specification

• Jaw position measured from the beam centerline to the 50%


isodensity line for each of the upper and lower jaws shall coincide with
the independent jaw digital position displays to an accuracy of ±2 mm
at 100 cm TSD.

Test Setup
1. Level the Gantry at 0° (180° VAR).
2. Using a calibrated front pointer, set the Couch top to 100 cm TSD.
3. Tape a piece of accurately ruled millimeter graph paper to the Couch
top (or a precision ruler may be used)
4. Turn on the Field Light and align the graph paper (or ruler) to the
Clinac crosshairs.

Method of Test
1. Independently drive each jaw so that the 50% isodensity point of the
projected jaw shadow corresponds to the jaw positions listed in the
following table. Verify the digital readout displays meet specification.
2. Fill out the applicable boxes in the Independent Jaw Digital Readouts
table.

2.3.2 Symmetric Mode

Specification

• The projected light field size measured at the 50% isodensity line for
each jaw shall coincide with the symmetric digital position display to
an accuracy of ±2 mm at 100 cm TSD.

Test Setup
1. Use the previous test setup but select the Symmetric jaw mode.

Method of Test
1. Symmetrically drive the X & Y jaws so that the 50% isodensity point of
the projected jaw shadows correspond to the field size positions listed
in the following table. Verify the digital readout displays meet
specification.
2. Fill out the applicable boxes in the Symmetric Jaw Digital Readouts

2.5. Collimator Rotation Readout Calibration

Specification
• The true angular position of the Collimator shall coincide with the digital
display to an accuracy of ±0.5° and the mechanical scale on the Collimator
assembly to an accuracy of ±1°.

Test Setup

The following steps provide one method of verifying the true angular
position of the Collimator. Other methods can be used as long as
the same end result is achieved.

1. Position the Gantry to 90° or 270° and the Collimator to approximately 0°


(180° VAR).
2. Place the top of the Couch near isocenter and open both sets of jaws.
3. Place a level on the Couch top so that the light field will project a shadow of
the level on the treatment room wall. Use shims to level the level.
4. Turn on the light field. While observing the shadows cast by the lower jaw
edge and upper edge of the level, close the lower jaws and adjust the
Collimator position until both shadows are parallel. This will be the
reference for the 0° (180° VAR) collimator position.
5. Rotate and level the Gantry to 0° (180° VAR).

Method of Test

With the Collimator mechanically aligned to its center position, it is


advisable to center the Couch rotation for the next section at this
time. Turn on the light field and rotate the Couch until the projected
edge of the Couch top (as observed on the turntable) is parallel with
the projected edge of the Y2 upper jaw.

1. Without moving the Collimator, Verify the Collimator digital and mechanical
readout displays meet specification for the 0° (180° VAR) position and
check off the applicable box in the following table.
2. Close the jaws to 20×20 cm and align the radial (inplane) crosshair line to a
sheet of graph paper placed on the Couch top at 100 cm TSD.
3. Rotate the Collimator to 90° until the crosshair line is again aligned with the
graph paper. Projected crosshair lines may not be perfectly perpendicular
so use the same crosshair line during these tests. Verify the digital and
mechanical readout displays meet specification.
4. Repeat the test with the Collimator rotated to 270º.
5. Fill out the applicable boxes in the Collimator Angle Digital & Mechanical

6. Re-position the Collimator to the true 0° (180° VAR) position for the next
test.

2.6.2 Couch Longitudinal Readout

Specification

• The Couch longitudinal travel shall coincide with the digital display to
an accuracy of ±1 mm.

Test Setup
1. Replace the wooden service panel with the carbon fiber panel.
2. Install the Varian provided tape measure into the PRO Alignment Tool
so that the 140 cm mark on the tape measure lines up with the sight
window scribe marks.
3. Install the Longitudinal PRO Alignment Tool onto the Couch carbon
fiber top at the 0 position index. The end of the tape measure should
be extended toward the Gantry PRO display.
4. Position the Couch to 0° (180° VAR) and 100 cm TSD.

Method of Test
1. Gently support the end of the tape measure to keep it level and float
the couch top until the crosshair aligns with the 20 cm mark. Verify
the digital display meets specification in the following table. Note: this
will be the 0cm position for scale convention IEC 1217).
2. Float the couch top until the crosshair aligns with the 150 cm mark on
the tape (press down on the tape to keep it flat and bend the end of
the tape down to avoid hitting the PRO display). Verify the digital
display meets specification.
3. Fill out the applicable boxes in the Couch Longitudinal Digital

1. Using the same PRO Alignment tool setup from the previous test,
center the Couch top laterally by aligning the crosshair to the scribe
mark on the PRO alignment tool.

Method of Test
1. With the couch aligned to the center travel position, verify the digital
display meets specification for the central position in the following
table.
2. Move the tabletop 23 cm to the right by measuring the distance
between the crosshair and the PRO tool scribe mark. Verify the
digital display meets specification.
3. Repeat the test with the tabletop moved 23 cm to the left of center.
4. Fill out the applicable boxes in the Couch Lateral Digital Readouts
table.

There are other methods to demonstrate this test, but this


method accurately measures the true position of the tabletop
surface relative to isocenter.

2.6.4 Couch Vertical Readout

Specification

• The Couch vertical travel shall coincide with the digital display to
within ±1 mm.

Test Setup
1. Position the Gantry to 0° (180° VAR).
2. Using a calibrated front pointer, position the tabletop to isocenter with
the carbon fiber panel installed (do not use the wood Service panel).
Remove the front pointer after positioning.

Due to couch top panel flex, it is recommended to calibrate


the couch vertical position referenced to the center of the
couch top near the 0 position index notch. However, the
calibration position must be consistent with any other Clinacs
already installed on the site.

3. Using the provided tape measure, extend and slide the tape measure
between the Couch top and the side support rail (at any location on
either side of the Couch). Tape the end of the tape measure to the
bottom edge of the Couch. End result:
• The tape measure dispenser should be sitting on top of the side
Couch rail
• The tape measure should be extended downward between the
Couch top and the side rail
• The end of the tape measure should be hooked under the
bottom of the Couch and securely taped to the base of the
Couch.
4. Record the tape measurement for the “top edge” of the Couch side
rail. This will be the measurement reference value for 100 cm TSD.

Method of Test
1. With the couch in the test setup position, verify the digital display
meets specification for the 0 cm (100 cm VAR) position in the
following table.
2. Rotate the Gantry 50° in either direction to allow full couch extension.

2.7. Optical Distance Indicator (ODI) Calibration

Specification
• The ODI rangefinder shall indicate the Target to Surface Distance (TSD) to
an accuracy of ±1 mm at 100 cm TSD and to an accuracy of ±5 mm for all
other distances.

Test Setup
1. Use the same test setup from the previous test.

Method of Test
1. Turn on the Field Light and project the crosshairs and ODI rangefinder
display on a piece of white paper. Verify the ODI meets specification for the
100cm position in the following table.
2. Using the same tape measuring technique in the previous test, sequentially
drive the Couch 20 cm above and then 30cm below the 100 cm reference
position. Verify the ODI display meets specification. It will be necessary to
select “Raw Scale” readouts in Service Mode in order to raise the Couch
above isocenter with the Gantry at 0°(180° VAR).
3. Fill out the applicable boxes in the OBI Optical Display table.

If MLC is installed, also complete the rangefinder measurement

3. RADIATION ISOCENTER TEST

3.1. Gantry Rotation Spoke Shot

Specification
• Central axis X-ray beam variation due to Gantry rotation shall be confined to
a sphere of ≤1.0 mm radius for counterweight systems and ≤2.0 mm radius
for Beamstopper systems.

Test Setup
1. Set the Collimator angle to 0° (180° VAR).
2. Open the upper jaws to 40 cm.
3. Using the Independent jaw mode, close both lower jaws to 0.5 cm so that a
symmetrical 1.0 cm field is projected relative to the crosshair. The lower
jaws must be positioned equidistant to an accurately aligned crosshair for
this test to be valid.
4. Using support blocks (such as solid water slices), position an X-ray film so
that it is standing vertically on the Couch top and perpendicular to the lower
jaws. If using solid water slices, ensure that the slices will not fall!
5. Position the Couch height so that the center of the film is near 100 cm TSD.
Position the Couch lateral so that the approximate center of the film is
aligned to the Clinac crosshair.
6. Rotate the Gantry throughout its full rotational range and verify there will be
no collisions with the couch setup.

Method of Test

It may be necessary to experiment with exposure depending on film


type. X-Omat V film normally requires approximately 130–150 MU
and TL Film requires approximately 12-15 MU for this test.

1. Rotate Gantry successively from 90°, 0° (180° VAR), 275° (265° VAR), and
185° (355° VAR) while exposing the film at each position. Develop the film.
2. Bisect the 1.0 cm exposures in the center with a sharp line.
3. At the intersection of lines, measure the longest line of the trapezoid. This
represents the diameter error. Refer to Figure 1.
4. For retractable Beamstopper systems, repeat the test with the Beamstopper
extended.

3.2. Coincidence of Light Field and X-ray Field

Specification
• The field defining light shall coincide with the 50% isodensity line on an X-ray
film at 100 cm TSD within the specifications listed in the following table.

If large film is not available, this test can be demonstrated with a


20×20 cm field size. Write the selected field size in the table.

Test Setup
1. Machine positioning:
o Gantry 0° (180° VAR) and level
o Collimator 0° (180° VAR)

1. Tape a piece of X-ray film to the top of the Couch.

4. Repeat for Hi-X.
5. Develop the films and compare the 50% isodensity lines of the X-ray field
edges to the field light edges.

4.1. Photon Energy Depth of Ionization

Specification
• The maximum buildup and intensity at 10 cm depth shall meet the
specification shown in the Photon Depth of Ionization Table.
• All Photon Depth of Ionization tests are specified using a water phantom at
100cm TSD and a 10×10 cm field size.

Extra care should be used when setting up for depth of ionization


scans. Errors in tank level, probe depth, probe correction offsets,
etc, will cause significant errors in the energy curves. All scans
should be started at max depth and plotted to the surface.
As a rule of thumb, it should never be necessary to adjust the BMAG
shunt values during installation. If measurement errors occur, verify
the scanning system setup. If in doubt, Dmax Diagonal scans can
be run and compared to the Factory Red Book values to verify if the
beam energy is correctly setup. These scans (referred to as Horns)
are much more sensitive to energy changes than a Depth of
Ionization curve.

Test Setup
1. Set up and level the water phantom for 100 cm TSD. Refer to the Wellhofer
Operation Manual for proper setup.
2. Setup the Wellhofer for Depth Dose scanning
3. Set the jaws to 10×10 cm.
4. Turn the PFN Servo ON and the Dose Rate Servo OFF.

1. Beam on in Lo-X, Rep Rate 3 and scan the central axis depth of ionization.

4.3. Photon Field Flatness and Symmetry

Flatness Specification
• The arithmetic average between minimum and maximum X-ray beam
intensities within the central 80% of the radial and transverse major axes at
100 cm TSD and at a depth of 10 cm, shall not exceed the specifications
listed in the X-ray Beam Symmetry & Flatness tables.

Symmetry Specification
• The maximum variation in integrated dose between any two corresponding
points equidistant from the centerline within the central 80% of the radial and
transverse major axes at 100 cm TSD and a depth of 10 cm, shall not exceed
2.0%.

Varian-provided Wellhofer scanning systems utilize the Varian


protocol for analyzing beam symmetry. This protocol actually
compares equidistant points on each side of beam centerline and
displays the asymmetry between the two worst points. As a result, a
small bump in a scan or a central offset error can result in large
asymmetry values. For this reason, it is very difficult to achieve very
low symmetry values for both the large and small field sizes. As a

rule of thumb, if the asymmetry measures 1.0% on a Varian


Wellhofer system, a customer scanning system analysis (which
typically utilizes a symmetry averaging protocol) will typically be
<0.5%. If necessary, this can be demonstrated by switching
between the Varian and IEC protocols in the Wellhofer Buddleship
scanning software.

Test Setup
1. Level the Gantry at 0° (180° VAR).
2. Setup the Wellhofer scanning system for INPLANE scanning. (Refer to
Figure 2)
3. Level the tank in both planes.
4. Using the front pointer set the top surface at 100 cm TSD.
5. Open the jaws to 40×40 cm.
6. Set the probe position to the zero position (using the 0 hot key) and the
depth to 1 cm to allow visual probe alignment to the Clinac crosshairs (the
crosshair should bisect the probe tip).
7. The reference probe should be positioned as far as possible away from the
scanning path so that it is does not cause scatter that could affect the scan
results. (Refer to Figure 2 for a diagram of scanner set up.)

Method of Test
1. All scans should be saved with the following information: Energy, Scanning
Plane, Jaw Size and Date.
2. Scan through the X-ray field central axis at a depth of 10 cm in the radial
plane (Inplane) for each energy at field sizes of 40×40 cm and 10×10 cm.
Radial scans should always be performed prior to transverse (Crossplane)
scans in case any steering adjustments are required.
3. Analyze the scans using the definitions described in Figure 3 & Figure 4,
and record the data in the following table (and in the SVS if not already
completed).
4. Setup the Wellhofer scanning system for CROSSPLANE scanning and
repeat the scans in the transverse plane. For Buddleship tanks, rotate the
couch to 90º or 270º.
5. Analyze the scans and record the data in the following table (and in the SVS
if not already completed). Enter NA in any boxes that do not apply.

4. Repeat the scans with a 10×10 cm applicator.
5. Repeat the scans for HDTSe with the HDTSe- applicator installed and a
36×36 cm field size. Use the applicable 6e or 9e probe depth shown in the
previous table.
6. Analyze the scans using the definitions described in Figure 3 & Figure 4,
and record the data in the following table (and in the SVS if not already

7. Setup the Wellhofer scanning system for CROSSPLANE scanning and

4.5. Photon and Electron Symmetry Interlocks

Specification
• The asymmetry interlocks [EXQ1/EXQ2] shall terminate the beam at or
before field asymmetry exceeds 2.0%.
• Photon asymmetry is measured at the 25% and 75% points of the full width,
half maximum value for a 40 cm square field.
• Electron asymmetry is measured at the 10% and 90% points of the full width,
half maximum value for a 25 cm square field.

The Console Symmetry meter indications (R SYM & T SYM) are


carefully calibrated in the Factory so that 1.0 on the Console meter
corresponds to 1.0% (±0.05) beam asymmetry. Since it is difficult to
accurately demonstrate the meter calibration in the field, the factory
calibration data will be used to verify the meter calibration.

Test Setup
1. Select the Clinac Service Mode and override DOS1, DOS2, DS12, XDRS,
TIME, and EXQ1 & EXQ2 interlocks. Turn the Dose Rate Servo ON.
2. Open the radial and transverse Angle Steering Servo Loops (on B15 &
B16).
3. Verify the Position Servo Loops are closed.
4. Connect one end of a jumper to B19-TP8 (leave the other end
disconnected)
5. Select Lo-X. Measure and record the ANG R & ANG T programming values
in the following table. Repeat for Hi-e.

Method of Test

The following steps are written for the Radial symmetry tests. Refer
to the words in parenthesis ( ) when performing the Transverse
symmetry tests.
Always use the Setup – Set All menu selections when changing
energies.

1. Beam on in Low X-ray and adjust pot R54 on B19 so that the voltage at
B19-TP8 measures the same value as the Lo-X ANG R (ANG T)
programming pot voltage.
2. With the beam still on, connect the jumper wire between B19-TP8 and the
ANG R (ANG T) test point on the front of the Program PCB.
3. Adjust B19-R54 until the R SYM (T SYM) meter on the Console monitor
indicates < 1.0. If the EXQ1 (EXQ2) interlock is red, clear the interlock by
depressing the “I” & “D” keys.
4. Slowly adjust R54 in either direction until the EXQ1 (EXQ2) interlock trips.
Immediately note the R SYM (T SYM) meter display.
5. Multiply the trip value by the Lo-X Angle Servo calibration factor in the Red
Book to determine the amount of beam asymmetry when the interlock
occurred. Verify the calculated value meets specification.
For example: If the R SYM meter indicated 1.72 when the EXQ1 interlock
tripped, and the Lo-X Radial Angle Servo calibration value in the Red
Book is 0.98%, the actual beam asymmetry when the interlock occurred
would be: 1.72×0.98 = 1.69% (rounded off)
6. Repeat these tests for the Hi-e energy.
7. Fill out the applicable boxes in the Symmetry Interlocks table.
8. Remove the jumper to restore all original steering values. Clear all
interlocks, and close the Angle Steering Loops.

If time permits, complete these tests for all energies per the
requirements in the SVS, if not already completed.

4.6. Beamstopper Attenuation

4.6.1 Attenuation Factor

Specification

• The Beamstopper shall attenuate the primary X-ray beam by a factor


of 1000 (≤0.1%).

Method of Test
1. Set the Gantry at 90° or 270°. Position the Beamstopper to the fully
extended position. Set the jaws to 10×10 cm.
2. Using an ionization chamber and buildup cap, measure the central
axis radiation intensity 15 to 20 cm beyond the Beamstopper in the
highest X-ray energy.
3. Without moving the test ionization chamber, repeat measurement with
the Beamstopper retracted. The ratio of measurements shall be
≥1000 to 1.

through the treatment room walls in the beam path in X-ray
energies only.

5. DOSIMETRY
Dosimetry calibration was already performed in the factory. Depending on the protocol
used by the hospital, the absolute dose calibration should be relatively close. Therefore, it
is not necessary to calibrate the absolute dose during these tests. Instead, the dose
integration data shall represent relative data for the sole purpose of verifying the
specifications. The final absolute dosimetry calibration (which is ultimately the responsibility
of the hospital) should be performed by the customer after completion of the acceptance
testing.

IMPORTANT NOTE TO CUSTOMER: When calibrating the absolute dose,


the dosimetry system calibration should be adjusted so that 1 MU displayed
on the Console corresponds to the delivery of 1 cGy of dose at the depth of
dose maximum (Dmax) in water for a 10×10 cm field at 100 cm TSD.
Calibration in any other manner may compromise the reliability of the
system and the Clinac warranty as expressed in Varian’s Terms &
Conditions of sale

There are three choices for recording the test data for beam dosimetry tests.
1. Lotus Notes SVS Worksheet
2. Excel Spreadsheet Worksheet (available on the PSE web site in
Downloads 2, titled HE-CAP-Dosimetry Spreadsheet.xls)
3. SVS Worksheet (paper copy)
Use the worksheet in the Lotus Notes SVS, if available. Integration data is
entered in a provided worksheet and all test results are then automatically
calculated and entered in the required tables. This is the most efficient
method. After completion of the SVS, print a copy for the customer to
provide a permanent record for the Dosimetry section test results.
Use the Excel spreadsheet if the Lotus Notes SVS is not available.
Integration data is entered in a provided worksheet and all test results are
then automatically calculated and entered in the required tables. After
completion, the spreadsheet must be printed out, signed and attached to the
SVS and CAP paper documents. Once attached, it is necessary to write
“See Attached Spreadsheet” diagonally across each of the applicable
tables in the SVS and CAP and initial or sign the box below each table.
Only use the SVS paper worksheet if the other two options above are not
available. This method is very time consuming and permits possible errors
due to potential human calculation errors.

Definitions
RRmax refers to the maximum Rep Rate available on the Clinac iX being accepted

Test Setup
1. Tests shall be made using a Farmer Secondary Standard, or PTW ion
chamber (or equivalent), located in a water or suitable solid phantom. The
output of the ion chamber should be measured using an electrometer.
2. Accumulated dose and intensity should be measured at central axis at
100 cm TSD using the following depths and field sizes for all tests. Actual
Dmax depths are not required since these dosimetry checks are relative
readings.
3. Ambient temperature variations should not exceed ±5°C during the course
of these tests.
4. The Gantry should be leveled at 0° (180° VAR) for all tests except as
indicated in the Dose Reproducibility with Gantry Angle tests.
5. All Clinac servos must be ON (Dose Rate Servo, Steering Servo Loops and
PFN Servo).
6. Field sizes will be as follows:
o 10×10 cm for all photon energies
o 15×15 cm applicator for all standard electron energies
o 36×36 cm HDTSe applicator for HDTSe energy

1. Tests can be performed in Service Mode, but ALWAYS use the


Set Up - Set All sequence when changing energies. Do not use
shortcut selections.
2. Because the HDTSe- testing requires long runs, it is
recommended to perform these tests last to avoid potential
ionization issues of the test probe.

6. DYNAMIC THERAPY

6.1. Arc Dynamic

Specification
• The Dose Difference Standard Deviation (sD) shall be <0.20 MU.
• The Dose Weighted Gantry Position Difference Standard Deviation (sDP) shall
be <0.50°.

Test Setup
1. Position the Couch angle at 0° (180° VAR) and vertical position at isocenter.
2. Enter the treatment room and verify that no obstructions will prevent a
dynamic run.
3. Disable RV interface before performing dynamic tests or place VARiS
Treatment in Standby.
o Enter Physics mode. Select Configuration / RV Interface and disable
Interface Enabling.
o Save the changes before exiting Physics mode.

Method of Test

The dose deviation values will be displayed on the Clinical mode


screen at the completion of each dynamic treatment. The displayed
values should be compared to the specifications and each test
should be checked off if the deviation values meet specification.

1. Enter Clinical mode.


2. Run the Hi-X arcs as defined in the Dynamic Therapy table and verify the
deviation values meet specification.
3. If the E-Arc option is included, insert the Electron Arc applicators consisting
of the Electron Arc tray (inserted in the Interface Mount and an E-Arc
Aperture tray (inserted in the Accessory Mount).
4. Run the Hi e arc as defined in the Dynamic Therapy table and verify the
deviation values meet specification.
5. Fill out the applicable boxes in the Dynamic Therapy table. Enter NA in any

6.2. Enhanced Dynamic Wedge

Specification
• The Dose Difference Standard Deviation (sD) shall be <0.20 MU.
• The Dose Weighted Jaw Position Difference Standard Deviation (sDP) shall
be less than 0.15 cm.

Test Setup
1. In Physics Mode, commission the Enhanced Dynamic Wedge and disable
the ORNT I/L.
2. Rotate the gantry to 0º (180º VAR).
3. Disable RV interface before performing dynamic tests or place VARiS
Treatment in Standby.
4. Enter Physics mode. Select Configuration / RV Interface and disable
Interface Enabling.
5. Save the changes before exiting Physics mode.

Method of Test

The dose deviation values will be displayed on the Clinical mode


screen at the completion of each dynamic treatment. The displayed
values should be compared to the specifications and each test should
be checked off if the deviation values meet specification.

1. Enter Clinical Mode.


2. Run the Enhanced Dynamic Wedge entries listed in the following table and
verify the deviation values meet specification. For the Hi-X partial treatment
(the last test), press F4 when MONITOR UNITS is displayed to enter 95 out
of 100.
3. Fill out the applicable boxes in the Enhanced Dynamic Wedge table.

Decommission Enhanced Dynamic Wedge and enable the ORNT I/L


option in the Physics mode after the completion of this test. EDW
summaries can be viewed or printed from the Communications Mode.
The equations used to compute the standard deviation values can be
found in the EDW Implementation Guide.

7. LASERGUARD COLLISION PROTECTION SYSTEM


LaserGuard provides an infrared protection zone below the Collimator to prevent Gantry
collisions with patients and objects.

7.1. Protection Zone Area Verification

Specification
• With the Collimator & Gantry at 0° (180° VAR), the protection zone area is
centered and parallel to the Interface Mount and meets the following
requirements:
o The 177.8-mm handle protrusion of the T shaped Gauge (100012420-
01) will clear the protection zone (refer to Figure 5).
o The 36.9-mm Gauge Plug (100011752-02) clears the protection zone.
o The 1-mm Gauge Shim (100011752-04) used with the 36.9-mm Gauge
Plug will penetrate the protection zone on either side of the zone.
• With the Collimator at 90° and the Gantry at 0° (180° VAR), the protection
zone tilt alignment is 3° relative to the Interface Mount (ref Figure 7) and
meets the following requirements:
o The 26-mm Gauge Plug (100011752-01) attached to the rear Interface
Mount pin (gun side) will clear the warning zone.
o The 47.8-mm Gauge Plug (100011752-03) attached to the front
Interface Mount pin (target side) will clear the warning zone.
o If a 1-mm shim is added to either Gauge plug, the warning zone is
penetrated.

Method of Test
1. Position the Gantry and Collimator at 0° (180° VAR).
2. Using the T-shaped alignment gauge device provided with the system,
Verify the protection zone is centered relative to the Interface Mount. Install
the device as shown in Figure 5 and confirm that the red indicator light on
the Gantry PRO display is not illuminated.

3. Adjust the volume on the Master Control Panel (in the Stand) so that it is
audible to the user in the room.
4. Attach the medium-height gauge plug to the Interface Mount over the
Accessory Mount alignment pin as shown in Figure 6. This gauge plug
should not intrude into the protection zone and the red indicator light should
not illuminate.
5. Place the medium gauge plug onto the opposite alignment pin and Verify
the red indicator light does not illuminate.
6. Reattach the medium height gauge plug at each location with the 1-mm
shim in place. The red light should now illuminate and there should be an
audible alarm.

7.2. Protection Zone Tilt Verification


1. Rotate the Collimator to 90°.
2. Attach the longest and shortest gauge plugs as shown in Figure 7. Neither
of these gauge plugs should cause the yellow light on the Control Panel (in
the Stand) to illuminate.
3. Add a 1-mm shim to the shortest gauge plug. The yellow light should
illuminate.
4. Remove the 1-mm shim and add it to the longest height gauge plug. The
yellow light should illuminate.

7.3. Motion Stop Function Verification

Specification
• With the treatment room door closed, the Gantry motion should stop when an
intrusion into the protection zone occurs.

Method of Test:
1. Remove the Beam-On key from the Clinac Dedicated Keyboard. While
remaining inside the treatment room, close the room door to apply –12V
from the Door switch to the LaserGuard control unit.
2. Place the foam collision block provided on the Couch top in a position that
will cause the Collimator to hit the block before it hits any portion of the
Couch (see Figure 8). Make sure the corner of the block is positioned so
that the corner will penetrate the protection zone and not hit in the central
clear zone area (see Figure 5).
3. Using a pendant, carefully rotate the Gantry towards the block. Verify that
Gantry motion stops when the block penetrates the protection zone and that
the red collision indicator on the Gantry PRO is illuminated.
4. Exit the treatment room, close the door and verify the red indicator on the
Console display is illuminated.

7.4. Power Key Switch & Override Function Verification

Specification
• The yellow indicator light on the Gantry PRO display and at the Console
display should illuminated when the power key switch is off or either of the
override buttons in the Stand are activated.

Method of Test
1. Turn the key switch to the OFF position. Verify that LaserGuard is
deactivated, and that the yellow indicator on the Gantry PRO and Console
display is illuminated.
2. Re-enable the LaserGuard system by turning the key switch to the ON
position. Press each of the override buttons to ensure the yellow indicators
are illuminated.

8. CLINAC OPERATIONAL TRAINING


Basic Clinac operational and safety training may be required for the Physicist(s) and/or
Therapist(s) at the end of installation. Application training will be provided at a later
scheduled date. To ensure the best possible training session, provide copies of this
document to all trainees at the start of training so notes can be made for their future
reference. In addition to the items in this document, the Physicist(s) also requires basic
Service Mode & Physics Mode training. Service Mode training can be completed during
Acceptance testing. If the site does not have a supporting staff, Applications personnel will
complete training.
