AFFIDAVIT FOR DELAYED REGISTRATION OF BIRTH

I, LILY BELLIE BITCHER, of legal age, married, Filipino and with residence and
postal address at Doña Ma. Village 2, Punta Princessa, Labangon, Cebu City, after
having been duly sworn to in accordance with law, do hereby depose and say:

1. That I am the applicant for the delayed registration of my birth;

2. That I was born on March 11, 1959, in Baracatan, Toril, Davao City, Philippines;

3. That I cannot remember the name of the person who was attended my birth;

4. That my parents were ATTO BELLIE and ALLONG ALAYO, whose marital status
I cannot determine since there is no record of their marriage in the Local Civil
Registry in Toril, Davao City;

5. That I am married to CELESTINO BITCHER;

6. That the reason for the delay in registering my birth was due to my parents’
negligence;

7. That I am executing this affidavit to attest to the truthfulness of the foregoing

statements and for all legal intents and purposes.
