4. Will the COVID-19 test cost at private facilities be part of my outpatient / hospitalization & surgery (H&S) entitlement limit?
Depending on the provision. Please refer to slide 2-4 for further details on the provision and scenarios .The COVID-19 test cost at private facilities may be
included in the employee outpatient limit for scenarios classified as Personal.

6. The private hospital imposed for me to perform COVID-19 test in order to accompany my wife to labor room for delivery. Will the Company
cover the cost?
Yes, COVID-19 test imposed by private hospital as pre-requisite for admission is covered for the patient and accompanying dependant/employee
themselves . Cost deducted from employee’s Outpatient annual entitlement
7. Will the Company cover test cost if I perform the test at private facilities more than one time?
Yes , based on the eligibility criteria.

# Category Definition
I Suspected Case of COVID-19 A. A person who meets the Clinical and Epidemiological criteria (refer table X)
B. A patient with severe acute respiratory illness:
SARI: acute respiratory infection with history of fever or measured fever of >38oC; and
cough; with onset within the last 10 days; and requires hospitalization.
ii Probable Case of COVID-19 A. A person (alive or dead) with a positive RTK-Ag.
B. A suspected case with chest imaging showing findings suggestive of COVID-19 disease
C. A patient who meets clinical criteria above AND is a contact of a probable or confirmed case or
linked to a COVID-19 cluster.
D. A person with recent onset of anosmia (loss of smell) or argeusia (loss of taste) in the absence of
any other identified cases.
E. Death, not otherwise explained, in an adult with respiratory distress preceding death AND was a
contact of a probable or confirmed case or linked to a COVID19 cluster (not relevant for this
guide)
Iii Confirmed Case of COVID-19 A. A person with a positive RTK-Ag in pre-determined areas/locality with prevalence of COVID-19 >

B. A person (alive or dead) with a positive molecular test (RT-PCR or rapid molecular).
iv Person Under Surveillance Asymptomatic individual subjected to Home Surveillance Order
