(Reconfirmation) Ensure that all employees are familiar with the 15 basic safety work rules

I would like to express thank everyone for your understanding and cooperation in our safety activities.

In response to the frequent occurrence of "occupational incidents including serious incidents" and
"serious near misses" in June and July 2022, I worry the current safety status of the Komatsu Group are
in a "extremely dangerous situation" and have imposed a "State of Emergency Komatsu Groupwide" by
"S&H 22-096" dated July 19, 2022.

In addition to this declaration, from 25 July to the end of September as I stated in my previous letter on
19 July, in order to normalize the current safety situation, once again I request that we thoroughly
strengthen the following "15 Basic Safety Work Rules" not only within the company but also at
subcontractors and related on-site contractors.

However, as shown in the attached "Fatal Accident Report", on August 3 (Wednesday) at KSC (China),
an accident occurred in which a KSC employee was hit by a contractor's truck traveling on the KSC
premises. And after that, the employee received intensive medical treatment, but unfortunately passed
away.

In response to the occurrence of this fatal accident, I sincerely wish to ask all of you to further strengthen
the "15 Articles of Basic Safety Operations," including those of the Komatsu Group's subcontractors and
onsite contractors who have already been requested to do the same.

In order to prevent any such tragic incidents from occurring again, I earnestly hope all of you to make
sure that your subordinates and other employees in the vicinity are fully aware of the importance of "never
neglecting 15 Basic Safety Work Rules" to prevent such incidents from occurring again.

【15 items of basic safe work】

1. Workplace must be well arranged and clean.

2. Keep checking safety of your work and surroundings.


3. Do not put a part or all of your body in dangerous area.

4. Constantly check and maintain equipment, tools, and protective equipment, and keep them in good

condition.

5. Do not use equipment, tool, or protective equipment, which is abnormal for breakage, failure, aging,

etc.

6. Observe “3 trouble processing rules" (Stop, call, wait) when abnormal state occurs in equipment or

procedure.

7. Standardize work (Clearly show work procedure, including equipment, tools, and protective

equipment).

8. Follow decided "work procedure" and use specified "equipment and tools" and "protective

equipment".

9. Be sure to perform "KY before work" when starting work or non-routine work which are not

standardized.

10. Turn off electric power, engine power, etc. when servicing or repairing equipment or machine.

In addition, hang "Do not switch on" tag to prevent third person from turn on electric power or

engine power. (Remove key if it is inserted)

11. In group work with one or more workers, appoint supervisor and following his/her directions.

12. Sufficiently understand work method and functions and operations of equipment used before

starting work. Do not start work with insufficient understanding.

13. Each worker must have obtained or finished licenses, qualifications, or training necessary for work.

14. Allow only authorized people to enter work area. (Visualize or indicate work area)

15. Control physical condition (Do not work when out of condition).
